Penile Implant Replacement Complications

Hello All,

Short time lurker, first time poster. I am 35 years old and got my first implant installed when I was 23. This was due to a pretty bad car accident with trauma that caused blood flow issues. My first implant surgery was done at Vanderbilt and lasted about 11 years with 90% satisfaction rate (lost some size) and no surgery complications.

My implant stopped working late 2019 and I got a second implant installed on March 6th, 2020. This was done by Dr. Cohen at Johns Hopkins. It's been a little over 3 weeks and the healing is going really well except for what seems to be either swelling or hardened blood in my testicle. This essentially is pushing up my pump to sit a little higher in the scrotum and a lot more noticeable to the naked eye.

I'm no doctor, but I have a feeling it has to do with the use of a drain in my first surgery and no use of a drain in my second surgery. That's neither here nor there. My short time lurking here I understand that everyone preaches patience and I certainly have that in abundance. However, I'm curious, has anyone else gone through anything remotely similar? It almost feels the pump is essentially sitting on top of a really full spot and it has no where to go but up. I swear that the stuff below was squishy and now hard, but the large amount of swelling I had certainly could have caused me to misinterpret what I was feeling.

I have my first follow up tomorrow but just wanted to see if there's anyone here that can share a similar experience. I'm assuming it's going to be awhile before whatever it is gets reabsorbed by the body, just hopefully not months and months.

Hello FMLFML85,

My doc told me that he would be putting in a bigger implant and that I would be able to recover most of what I lost. I am going to start cycling tomorrow so I can report with more accuracy the veracity of his statement over the next couple weeks.

My surgeon would not even discuss what size he would be implanting other than to say he would only install whatever length my anatomy called implied. He did say over sizing caused problems that no sane man would want to endure. That was the end of our conversation on pushing the envelope. I had my implant on March 26,2020. No problems at this point of which I am thankful.
66 yr old male married 36 yrs use trimix four yrs, cilais and Viagra. trimix work well developed scarring on both sides had implant 1/9/2020 at UT Med Ctr, Knoxville, TN Dr. John Lacy.

Welcome to the forum, smithfield.

I think what FMLFML85 was asking was, "what was the size of your original implant and what is the size of your replacement " Often an implant a centimeter or two longer than the original is placed when a revision is done. Whether that translates to an actually longer penis is open to question. Some of the extra length might wind up being embedded in the pelvic crus (more room being left at the removal of the failed implant) and some length consumed by tissue compressed in the glans. What is not consumed by those extra spaces (if they actually exist) would be seen as a bit longer external penis length.

If your body does not reabsorb the mass you describe, it should be a fairly easy and relatively non-invasive procedure to open your scrotum and take it out. A LOT less trouble than the implant surgery! Probably simpler than a vasectomy, too, which is easy enough. Recovery time will be a lot less, bruising, too. Incision healing might be the same length of time, but it might be able to be done laparoscopically. That would just be a small hole-punch and two or three small stitches. (I am no medical professional, so take my words as those of a simple layman.)

Thanks for the comment Lost Sheep. To the first part of your post, to be honest I don't remember how big the first device was, so sorry about not being able to provide much info on that.

I appreciate the second part of your post. I had a post op appt today with my doc who said pretty much the same thing. There's some possible concern of skin adhesion due to the mass "pushing" the device against my scrotum, so he gave the normal advice on tugging and hot baths/showers. I have a telehealth follow up at the end of april.

Altogether, it's not the best news ever, but it's not absolutely terrible. I'm hoping by the time late april comes around everything will be solid. I have some stricture (scar tissue) in my uretha from the trauma that caused my ED so I really really want to minimize the amount of catheter use; but if they have to go back in, I guess they have to go back in.

Just a quick comment. My first surgery was with a preconnected unit. The tubes were much too short so my pump rode high felt like it was stitched in place. AMS 700 LGX. Well after 4 months of fighting to push it down and loosen it up I finally had a second surgery to add tubing and now it site very low and out of the way. Your condition sounds very similar. Look at your card and see if the stamp is over the top preconnected box. The blank box below indicates that it needs to be assembled.

Just a quick comment. My first surgery was with a preconnected unit. The tubes were much too short so my pump rode high felt like it was stitched in place. AMS 700 LGX. Well after 4 months of fighting to push it down and loosen it up I finally had a second surgery to add tubing and now it site very low and out of the way. Your condition sounds very similar. Look at your card and see if the stamp is over the top preconnected box. The blank box below indicates that it needs to be assembled.

March 2019 I had a titan put in with the jp drain removed after day 1.

Revision march 2020 with an lgx. Drain was in for 2 days. I had a lot less scrotum swelling in comparison from last year..
18 cm plus 1 rte titan installed March 2019. Revision March 2020 by Dr. Andrew Todd, Richmond KY. He replaced the titan with an AMS 700 LGX 18 cm cylinder plus 2 rte for 20 cm total length.

I'm having the same problem. The surgeon connected the tubes and the pump is too high (it sits where my right testicle should be). And the pump pushes my right testicle up to the top of my scrotum. Causes pain while sitting, and also during certain sexual positions....
