I went in wed morning for my revision. Prior to going in for surgery I was given 5 days of antibiotics along with 5 days of gabapentin. This was to get a jumpstart on the infection risk. I was also one of the last surgeries to be done at my hospital so I was lucky it was scheduled for wed.
My initial titan was installed on March 7 2019. That journey is detailed in other posts, but it went well and I was pleased with how the titan fixed my peyronies and I got most of my pre ed length back by January 2020. My issue started with hernia surgery in jan. After hernia surgery my titan no longer functioned. I initially visited the doc who installed my titan. He is about 3 hours away and he said he could just remove the reservoir and pump and leave cylinders alone.
I was referred to a new urologist by my general practitioner to get another opinion. He ordered a ct and after reviewing it said it would be best to have the titan removed completely. He would replace it with an lgx. In addition, in removing the reservoir and tubing, a general surgeon might be needed if there were any additional issues with the hernia. I was very comfortable with the new urologist and he put me very at ease. I decided for the revision to go local and put my faith in his hands. He would also be able to coordinate with the general surgeon if there were additional hernia issues.
Preop I had about 1.5 hours of iv antibiotics before I was taken back for surgery. I prefer to be knocked out so I was out of it until I woke up in recovery. Of course when I did wake up I was in mummy wrap with a catheter and JP drain in. Surgery was over 3 hours so a little longer than I had hoped. The doc came in to visit me and told me he was able to upsize me with 18 cm cylinders plus 2 rtes for 20 cm total length. I was pleased and he was pleased. He had told me he would try to get me as long of cylinders as he could and keep rtes to minimum. He did that. I was within a half inch of my pre ed length with my titan so was pretty close with it. I was not expecting more than 1 cm increase for this revision if I could handle that even. I did not want any erosion issues. I am happy with this 20 cm length and see what happens over the next 6 months.
The revision went well. Great I would say so far. I was home by 4 PM wed. I took a 5 mg oxy before I left the hospital. It was a short 20 minute drive home this year instead of over 3 hours. That was really nice. I could not tell to much about how things look as all I could see was the tip with a catheter stuck out of it. I was not in very much pain at all. Mainly just an ache from the catheter and drain. I keep those in the rest of wed, all thursday, and get it out friday morning at first follow up.
Wed night I got up around 7 and walked around the house for about 30 minutes to get the blood moving. Laid back down to stay flat on my back. I did not want to much blood pooling in my groin but did want to move around. Took my next 5 mg oxy at 10 before going to bed. Still no real pain but wanted to stay ahead of pain. Last year I was in the hospital this first night so still had access to morphine through an iv. I was really pleased at how low the pain level is so far for this revision.
Thursday was mostly spent laying down flat on my back. I cut back on the 5 mg oxy to about every 12 hours instead of every 6. Still no real pain. I would have no problem walking around at all except having to carry the catheter bag around with me. A little awkward getting up and down with the mummy wrap.
Friday morning went for my first follow up at the doctor. Time to get the catheter and drain removed. Not fun but over real quick. Once they were out along with the mummy wrap being off, my comfort level went way up. The doctor deflated me as much as he could. Not to bad at all. I thought it might have been worse since I have not pumped up since jan and figured my scrotum would be back to pre bulb pumping sensitivity. Nope. No real pain from him using the pump.
Wow. The difference I felt after the catheter and drain was out was amazing. Walking just fine. Wife said I was walking normal. Positive steps. No issue at all. Looking down I could not see any bruising at all. I was swelled up some on my fat pad but that could be due a pretty good incision I needed there. Penis normal looking as well as my scrotum.
No baths for 2 weeks. Scrotum incision is glued but groin one is not. I can take showers. Basically just gently pull on pump daily and massage scrotum. Till I see him again that is all he wants me to do. Wait to pump for now.
Easy car ride home. When we did get home i was walking fine. Made breakfast and stayed up for a bit, then went back to bed. Got a good look at my penis and scrotum. No bruising and no swelling yet. I can feel pump easily. Doc said he put it a bit more forward than my last one. I told him the titan pump was sitting a bit tight toward my prostate and made riding my atv uncomfortable. Will see if this position helps that activity.
Fyi, if I needed to I feel like I could have walked just fine through an airport at this time. Sitting in an airline seat I cannot comment on as I am waling around and laying back down. No problem getting up and down at all. I am wearing my penis laying up against my belly. I also am wearing tight boxer briefs with a soft wash cloth between it and the briefs to keep it from moving around much.
Day 2 is looking pretty good so far.

18 cm plus 1 rte titan installed March 2019. Revision March 2020 by Dr. Andrew Todd, Richmond KY. He replaced the titan with an AMS 700 LGX 18 cm cylinder plus 2 rte for 20 cm total length.

You mean, you got home and were not loopy? Damn, how much time did you have in recovery? And how much antibiotic did they even put you on prior to surgery? They did not do that to me when I had the infection, they just soaked me in peroxide and Betadine during the surgery!

I ave to say recovery for this revision has been so easy it scares me. I have almost no bruising. Swelling is almost gone. I am not even taking tylenol. So no pain.
Doc deflated me at day 2 so I am flaccid. Lgx tubes are still hard but definitely not close as hard and stiff as the titan.
No inflating for a couple weeks. I am currently instructed to gently pull down on bulb and massage scrotum daily.
I am anxious to pump up this lgx and see what it looks like.
20 cm equals about 7.9 inches. The 19 cm titan had me at just over 6.5 inches fully pumped. I am hoping this revision gets me closer to my pre ed 7.
