I am about 3 weeks, pre-surgery for an IPP (Inflatable Penile Prosthesis).

I've done a considerable amount of research about the human penis in the last 2 years due to the onset of ED (vascular leakage) and now, impending IPP surgery.

First, some very important facts and information:
1. Ageing causes deterioration of the interior of the penis. Lack of oxygen is the root cause of the problem which worsens with age (it causes penile scarring and nerve death). This one thing causes ED in virtually every older man. If you can arrest this process of deterioration, you can go a long way toward saving your love life.
2. Nightly, small doses of Viagra/Cialis can help maintain penile health and cause the return of nocturnal erections, which are essential for penile health. These 2 drugs cause arterial dilation which improves blood flow and oxygenation of the penis. After I started taking a small, nightly dose of either drug, I began to have nocturnal erections again, which I had not experienced in 5-10 years.
3. Documentation available on this forum and written by surgeons explains the positive results that pre-surgery penis pumping produces. After daily penis pumping for 3 weeks, I have to relate my experience:

Pumping pre-surgery, I've had about 3 weeks to perform daily pumping and the results have been pretty amazing. My girth has significantly, visibly increased when either flacid or erect. My erections are a measured 3/4 inch longer. I did not really expect anything like this. If I'd known, I'd have been using a pump years ago.

My glans is significantly, visibly bigger. My wife recently remarked, "I don't know if I can get my mouth around it."

I purchased a $300 pump that is designed to be used with water, not air. A lot of paraphenalia, too complicated, although it worked, I am sending it back for a refund (it has a 60 day trial period).

I have an old air pump ($39) that I started using with water (shower, bath, spa, all work well). The $39 wonder pump works better than the $300 pump. But keep reading; there's more you need to know.

The big breakthrough was using water. Man, does water work well. It increases the vacuum strength without the usual leakage problems associated with using air. Once I get a good seal, I can increase the vacuum pressure to the point I think I could rip my dick off with it. I have to be careful not to injure myself with really high vacuum pressure. When my penis starts to sting and hurt a little, I stop pumping.

The key piece of equipment that has made using water so effective is a "cushion ring". It's allowed me to get a good fit, a good vacuum seal, greatly increase comfort, and the properly sized opening of the cushion ring prevents my scrotum and testicles from being sucked into the vacuum chamber at high vacuum pressure. Without a cushion ring, I would not have achieved such impressive results.

I only use one cushion ring at a time, picking the one with an opening that most closely matches the current diameter of my penis.

For those of you who have already had IPP surgery, I know that almost all of you complain that your glans size is diminished. I don't know if you can safely use a penis pump, post-IPP surgery. I hope you can. I encourage you to talk to your surgeon about using one before attempting it. You would hate it to damage the IPP. If you pump post-surgery, please send me a message telling me about your experience and results.

I only use the pump while showering for about 10 minutes per day. I pump, engorge my penis, hook the pump to a string around my neck so my hands are free, then shower, shave, and shampoo. The after effects of the enlargement last all day. When I urinate, my dick does not feel like my dick. It feels like a thick, meaty, sausage-like appendage that I don't recognize when I touch it. I really like it.

It's also plenty sensitive. No loss there.

I wish I knew this earlier too, as I am scheduled this Monday for surgery.
Let me ask you this: you pump up your cock, and leave it pumped for the 10 min. duration, or is this in increments, for a total of 10 min.?

I used to pump and release 10-30 seconds at a time for two 30 minutes sessions per day. Did that for months as I was searching for a surgeon and getting authorization for the implant.

60 days is the time length recommended by a couple of studies I read, but the most gains occurred in the first 20-30 days. After that, size increases/recovery slowed down. But continued. No real evidence accrued suggesting that vacuum therapy over such a short time as 60 days could provide an increase over a man's historical size, but size recovery does seem to be a real potential if the size loss is not too far distant in the past.

I leave my penis vacuumed and engorged (as much as I dare), for about 10 minutes every morning. Only when I'm ready to wash my groin do I release the vacuum and remove my penis from the vacuum chamber. This has worked really well for me.

I have read many different instructions on how to use a penis pump. Some say, 2 minutes of vacuum, 2 minutes of no vacuum. Some say 5 and 2 minutes, etc. I finally read one I liked written by a urological physician who recommended 10 minutes of steady vacuum per day. That recommendation worked for me so I adopted it.

Details, to eliminate ambiguities:

Step 1 - I slide my penis through the cushion ring opening, in its middle, and pull my penis fully through it. I push the cushion ring firmly against my torso, ensuring that my testicles are clear of it and not underneath it. I fill the vacuum chamber with warm water, then insert my penis into the vacuum chamber opening and start pumping. This inhales my penis into the vacuum chamber and starts to seal the vacuum chamber rim against the cushion ring which I ensure is properly aligned. I take a minute or so to pump to create the vacuum and get my penis mostly engorged. I am gentle when doing this for the first time in the morning. I allow my penis to gently, mostly, fill with blood.

Step 2 - I release the vacuum, remove my penis from the vacuum chamber, refill the vacuum chamber with warm water, reinsert my penis into it, and pump as I did in step 1 to recreate the vacuum, using the same technique and caution with the cushion ring as I did before. This time, I steadily engorge my penis as much as I dare. I do it to the point where my penis will not engorge any more. I have a little stinging and low level pain, mainly discomfort. No more. I wait a few seconds, make vacuum pressure adjustments, more or less, as necessary.

Step 3 - I have a looped string around my neck, sized long enough to hang just below my waist. I use the string to hook under and around the small neck at the top of the vacuum chamber where the vacuum tube is, and let my neck absorb and carry the combined weight of the hand pump, the vacuum tube, and the vacuum chamber which is full of water. This is comfortable and my hands are now free to bathe, etc.

Then, I begin my morning routine in the shower: bathe, shampoo, and shave my face. I take my time. This process takes about 10 minutes.

Lastly, I release the vacuum, remove the vacuum chamber, rinse it, and wash my groin.

The vacuum pressure almost always increases as I bathe, etc. I make vacuum pressure adjustments as necessary. I attribute this increase in vacuum pressure to the warm water in the vacuum chamber starting to cool. As the water cools, it shrinks, and this increases the vacuum pressure. If I start to experience pain, significant discomfort, I release a tiny bit of vacuum pressure. If the vacuum seal is somehow compromised and the vacuum pressure drops, I pump to reestablish a tight seal between the vacuum chamber rim and the cushion ring, then pump until the vacuum pressure is reestablished at the desired level.

Occasionally, I will have a 10 minute session at night in our spa. It just adds to the desired effect.
