As mentioned in a couple of other threads, at about 9 AM I said goodbye to the life of being a less than potent male. They installed AMS 700 LGX in me.

So now I am a swollen, sore, bruised but potent male with a nutsack that might hold Rhode Island. I’m Icing it good, so by morning maybe it’ll be down to the size an ice rink in Providence.

Enough of my lame humor. Here’s my question. I got the 18 cm LGX with 5 cm RTE.

Why would the doc do that? Wouldn’t it have made more sense to use a 21 cm LGX with 2 cm RTEs? You get the same length .
Robust, adolescent 65 year old. Venous leakage forever. Used shots, shots+pills 30+ years. Married to same wife ~35 yrs. Implanted 31Dec2019 in Austin Tx. AMS 700 LGX 18 cm with 5 cm RTE.

Most likely due to the need for the tubing to exit the cylinders more distally (closer to the surface, or buried more shallowly in the corpora). What I've read about the AMS cylinders, there is 4-4.5 cm of rear tip where the tubing comes out and the tubing comes out at the front of the rear tip, so about 4 cm from the proximal tip. If the tubing was 21+2, the tubing would come out of the cylinder at about 6 cm from the rear tip but 18+5 would have the tubing coming out around 9 cm from the rear tip.

Depending on where he chose to place the corporotomies (or had to place them due to your specific anatomy), he may have had to use the 18+5 arrangement to get the tubing exits closer to the surface.

Also, the extra 3 cm of the configuration you suggest (21+2), some of the expansion area could be in your perineum which isn't necessary for proper function. Also, the 21 cm would have more length expansion than an 18 if its max expansion is a percentage. That may not have been desirable with your specific anatomy.

There are any number of reasons the doctor chose that specific configuration. The only one who can honestly answer the question accurately is your doctor.

Thanks Wayne. That made sense.

As you can tell having general anesthesia today has boogered up my sleep pattern.

Your logic also sparked an idea that prior inguinal hernias (although one was fixed in 1959) might have caused some challenges in routing tubing and placing reservoir.

Right now I’m just sore and swollen and bruised and wondering what kind (read:how big) a penis I will end up with considering they put in 23 cm of hardware. I’m not greedy. Main thing is functional. The Doc left it at 55-60% inflated post surgery and it’s harder now than any erecion I’ve ever had. So I think we can check the functional block. I’ve run my fingers up and down and all around my pubic areas and there does not appear to be any numb spots of lessened feeling. But at the end of the day I’m still a guy and hope it’s about as big as before the surgery.

Thanks again. And a thanks to all for the encouraging words and good wishes as I approached being cut on oyesterday morning. Yawn.

HI. I had the same equipment installed on oct 31st. I do have an area of numbness on the underside of my penis but I can tell it is slowly resolving. At nine weeks, I still have some soreness in the area of the tubing above the pump block and the skin of my penis is still very sensitive in an irritating way. I am still unable to penetrate without pain but I understand that this is not so unusual. I can masturbate with lots of lube and a gentle touch. I did come out shorter and skinnier than before and I was quite disappointed with this. However, as I cycle twice daily and push the limit a little, I feel like I am making some improvements in those areas. I just wanted to let you know that this is a possibility and to try not to be too upset if this happens.

Please keep up the lame humor. Although it hurts the stitches in my nutsack when I laugh like I did reading your post, I love it. We must laugh as much as possible.

I think it depends a lot on the doctor. Some doctors try to use no or little RTE's. And I have seen others like in your case with 5cm or so RTE's. Like some doctor's do AMS or Coloplast only. Or some do infrapubic or peniscrotal only. And some deglove. Some use a drain and some don't. Just depends on the doctor.

Thanks for reminding me :shock: ... With all the implant surgery videos I've watched with clinical detachment, the only one that ever gave me a cringing or tingling in the crotch where it feels like my body is trying to suck my twig & two berries deep into my body, was when I saw and thought about the concept of having to be degloved. Just the thought of having the entire circumference of a penis being stripped down and the skin pulled back, for the surgical procedure still gives me a little cringe.

Thankfully, even though I had a fairly significant nickel-sized flat calcified plaque on the dorsal right side of my penis, mid-shaft, he didn't have to deglove. He simply remodeled and now that plaque appears to be free-floating between the tunica and facia.
