Ice and lying down for the first few days was very helpful for me. The catheter was the worst part of the whole experience. I'm exactly 4 weeks today. Just had my follow-up appt. with Dr. Kramer and he released me to go forth. I only need to see him again if there is an issue. Good Luck!

Hey All:

Well, the good news is I’m able to slide my laptop open and type a little bit. Promised some updates here’s where things stand…

Dr. Levine implanted my 27cm Titan Touch yesterday AM. Almost a 2 hour surgery at the ambulatory center, so this was longer than I anticipated based on some things I had read and videos watched. Staff was awesome. Dr Levine says all went excellent. My surgery was scheduled for 8am but they required me to arrive at 5:30a in order to get me on a strong antibiotic IV prior to surgery. Vancomycin over about 75 minutes.

My experience so far…

Pain
I’m a bit (knock on wood) surprised by the almost complete absence of what I would call “pain”. I’m pretty shocked by how mild it’s been thus far. They’ve warned me that more pain may set in over the next couple of days. Dr. Levine used lidocaine apparently fairly generously during surgery, so the thinking is that there’s some lasting effect of that anesthetic feeling post-op which I’m benefitting from. So all I can say at the moment is that I’m not having what I would describe as frank pain. I would describe the feeling as…a heaviness in my pelvis and groin, for sure…and a soreness...and I’m definitely aware that I’m very guarded about my mid-section/pelvis, just moving slowly and cautiously. But honestly - not really “pain”. We’ll see how this plays out. I was sent home with 5 days of oral antibiotics, a stool softener, and hydrocodone/acetaminophen for pain. I have only taken 3 of the Norco/Vicodin since yesterday AM. I don’t feel I need them. But I also understand that one principle of effective pain management is to not play catch up, ie, get a level of drug in your system and get ahead of the onset of pain, don’t try to play catch up once real pain sets in.

They sent me home with a scrotal drain. Not the most pleasant thing to have dangling off your operating-room-gauze mummy thong…but it did its job. Levine says about 1 in 3 patients go home with the drain. It filled almost all the way up by about 6-7 hours after surgery but then after emptying it, there was quite little additional blood/fluid captured by this morning. They had me go to the office at Rush this morning and based on how much fluid volume had been evacuated, they removed the drain.

I’m moving slowly and cautiously. Bending over isn’t fun. They want me mostly off my feet for the first 3-4 days. Ok to move around the house a bit but then get prone and lay down and try to stay down is the plan. Slept lousy last night but, again…not pain…just the generalized discomfort of a surgery, I think. It’s just hard on your body to get cut and be under a general anesthetic, regardless of the surgery type. So I feel sluggish but surprisingly good, considering what they did to me.

Peeing is not enjoyable. Feels like pissing shards of glass yesterday but even that is improving notably today. Who knows what happened while I was under…catheter I’m sure and from the videos I’d seen, there certainly seems to be a lot of crazy manipulation of the penis, so I shouldn’t be surprised my urethra was on fire I suppose.

Appearance
Honest to god, I’ve barely looked. I don’t particularly want to. My penis is buried beneath a mountain of gauze and wraps and I won’t remove those until Thursday, day #3. Interestingly, at the moment it is pointy and poking straight forward. Not erect. Not exactly. Dr. Levine’s PA said she just thinks it’s mostly the way they wrapped me up after surgery and that there’s so much gauze wound around everywhere down there that it seems like I have a small erection. I had to carry a jacket in front of me this morning going back to the clinic - would have frightened plenty of innocent passersby otherwise. For what it’s worth – Levine does not do early cycling of the cylinders during recovery like apparently some other surgeons do. His method is that he lets everything heal and settle down for 4 weeks. Then, at the 4 week re-check visit, usually then the hands-on tutorial takes place as to how to pump, how to deflate, etc. But apparently I’m not even partially inflated at present.

Sizing
As I said, Levine indicated he placed a 27cm Titan Touch. I literally have no idea what this is going to mean in terms of the finished result measurements once I’m healed and back in business. The night before surgery we did a bon voyage dose of my #13 TriMix injection and my wife helped me measure girth and length as accurately as possible so we would have a baseline understanding of what I started with. In this case the other night, with a big dose of TriMix (said the hell with it, last injection ever, and hit it with a full syringe of 100 units). Had a respectable erection that way and felt we got accurate measurements. I would say the rigidity and engorgement with this last dose of TriMix was at 90-95% of maximum erection. I measured 7.25” length and 6.25 girth. So, again, I guess we’ll see what happens in the final analysis. 27cm Titan length equates to 10.6 inches. This didn’t entirely make sense to me until Dr. Levine’s PA explained to me this morning that the implant does extend somewhat under/beyond the base of the visible penis length, and toward the pubic bone, so I’m really not sure how much of that 10.6” is tunneled under the base (and therefore not contributing to usable/visible length). I don’t particularly care. Dr. Levine told me going into this that, based on his examination of my flaccid penis, it was going to accommodate quite a large implant. So…yay for XL implants, I guess. My wife was with me yesterday at surgery and this morning at the clinic and I think it may be helpful and worthwhile maybe to share some of her thoughts here on this subject of sizing.

Communication
It’s all out in the open and it’s all transparent and we approach most everything, ED included, with eyes wide open and with a dialogue. So she had all the range of questions you can imagine about the entire process of managing ED from way back with PDE5’s through all the TriMix experiences and of course as the options were narrowing for TriMix and as more implant discussions came up between us, she had tons of questions. Will I have orgasms afterward, can I still ejaculate, etc. Tons of questions. But one thing which kept coming up, and not so much as a question, but as an actual articulated concern really, was she wanted to understand what might the differences in size be with the finished/functioning penis after implant vs her experiences with me prior. Specifically…she worried about girth and/or length being enhanced and bigger. This was interesting to me and unexpected. Definitely good to be told, and to hear her caution (ok, more than caution) that she worried if I would be too much for her afterward. The simpleton teenager still dwelling in most of us probably thinks some version of, “Wait - girls love big dicks and the bigger the better” and I more or less assumed my wife would be in that camp and intrigued by the bigger the better…because she’s definitely a penetration girl, she wants to feel FULL.

So I share this just to encourage anyone thinking of an implant to really, really talk through these kinds of concerns your partner may have. In our case, her perception has always been “omg you’re huge”. I don’t know how my dick compares, I just honestly don’t know and don’t think about this, but we’ve certainly run into situations where she is just…for lack of a better or more elegant phrasing…she’s a bit vaginally battered after sex. From both length/depth and width/girth as much as vigorousness. She’s a very athletic 52 years old. She feels tight like a vise to me and so I think she’s just anatomically small and she says - oh I don’t know about that, I just think you’re big. So I don’t know, guys…just have the discussion…calm her concerns. In our case, she knew it was called a “Titan” and the name carried with it a not-entirely-welcome perception that she was going to get obliterated by some superhero sized assault.

Her other main concern after she understood the process, mechanics of things, etc, was that she wondered if I am going to be maniacally trying to fuck her 24/7. She’s extremely sexual, but she shared her worry that, if I can just pump and go, and go effectively indefinitely, that she is not going to be able to keep up, and that we are going to potentially end up in a place where we resent behaviors in one another because my new found abilities go overboard and beyond what she can hope to keep up with. We had a great talk about this though. We learned that we both want the occasional sexual marathon exhaustion olympics, for sure…but that we both also want gentler, easier sex which comes with more frequency and more spontaneity. So I think we’ll just need to keep communicating and find that balance and for her she is going to need to trust me that I won’t go insane and break her and I’m going to have to earn and keep that trust.

I’m not sure what else at the moment. Hate to write a long novel here but I figure you never know what someone may be wondering about, so I just figured I’d share whatever I can think of about our experience here. I will post some photos soon to document the before, during, after.

Well, Day #3...definite pain...just generalized moderate pain across my groin. Was told yesterday by Dr. Levine's PA I should expect this over the next several days and she was definitely correct. Reaching for the hydrocodone and have been taking some, not full doses or at consistent intervals, but some. Going to take more to try and get ahead of the pain better, but still would say I'm shocked it's not been much worse. Probably a 4 on a pain scale of 0 - 10. Pain on urination is 90% gone now.

Day #4

Unwrapped everything last night on day 4 post-op and was allowed to shower. Interesting experience. I wanted all the bandages off and I desperately wanted a damn shower, but I found myself worried to really unwrap and see in cold hard light what the hell I had in fact done. Had visions of my balls swinging bruised between my knees, chainsaw massacre to my dick etc. Shockingly...none of that occurred. So I'm not always rational. Was advised not to physically wash anything down there but just to let soapy water run across. Felt no stinging or discomfort in doing this. Shaft has some increased soreness from day 4 to 5 here. Scrotum unchanged...just feels...heavy...full...sore, somewhat, but again I'm shocked it isn't worse. I honestly don't know how it's not worse. Posting some photos for anyone who might find value. So I guess here's the day 4 post-op reality. I've got to tell you, though, for what I know to have occurred under the knife, I remain pretty damn upbeat here. I'm sore. But I'm excited.
