Dr Eid has operating privileges at several hospitals including the out-patient surgical center by his office. Ask about using another hospital. Also ask if staying overnight in the hospital would change the situation.

I had a similar experience almost catch me. Dr Eid's office and the insurance said the procedure was 'approved' (NOTE: NOT the same as covered). However, the approval meant absolutely nothing. Before traveling, when I asked the insurance co point blank if there would be any portion I would have to pay, they said all of it since Dr Eid is out of network.
Dr Eid's office was as surprised as I was since they inferred that 'approved' meant paying for it. According to the insurance co, it is the patient's responsibility to determine if the Dr is in network as only the procedure was approved for coverage (IF the Dr is in Network!). (Then why is the request for approval Dr specific???)
I renewed my search for a Dr but the first qualification, besides expertise, was the Dr being in Network. Fortunately, Dr Kramer was!
