Without corporal dilation

Hey guys,

I came across a 2013 study ("Strategies for maintaining penile size following penile implant" if you Google it), which reports that penile-implant surgery in a group "without corporal dilation" led to 1.1cm improved Average postoperative penile-length; compared to a 1.2cm decrease in a group having it done "with corporal dilation".

When I asked my surgeon about this, I believe the science behind this lies in the fact that doing the procedure without corporal dilation better preserves the cavernosal smooth muscle (which also preserves the feeling of the erection after).

BUT, my surgeon tells me he DOES use some corporal dilation during the procedure.

So wanted to ask you brothers on Franktalk - have many of you had the procedure without corporal dilation, or is having the procedure done with corporal dilation much more commonplace? How feasible/unfeasible is it for the surgeon to be able to conduct the surgery without any corporal dilation?

I guess I don't have much choice if my surgeon is saying he will use some corporal dilation - but wanted to see how common a thing this was.

That study you quoted was co-authored by my surgeon.. he is awesome. :)

Hey Larry.. I’m new to the forum and trying to read as much of the posts as I can to acquire as much knowledge as I can. In terms of Dr Brock... Which above method did he practice with you? I am a patient of Dr Brock and am considering the implant more seriously now but trying to gain as much knowledge before I jump in.

Some dilation is necessary in order to insert the prosthetic cylinder into the tunica albuginea. One surgeon I interviewed indicated to me that his hospital's practice is to destroy or remove the tissue. That pretty much ruled out that option for me.

A related article referring to the advantages of pre-op vacuum therapy for a miniumu of 60 days prior to implantation is also informative. Pre-op stretching of the tunica seems to help with the implantation and minimizing trauma to the cavernosum tissue.

Thanks Lost Sheep. I will definitely review the info you have suggested. I have been doing vacuum therapy and require it plus tension rings to gain a hard enough erection to have sex with my wife. Not that big to begin with.. 3” flaccid and 5.5” erect so definitely want to keep that post op if the implant is the direction I choose to go. Definitely the more I read on this site the more I am confident that the implant is the way to go!

You're welcome.

Do not neglect the other ways sexual satisfaction may be achieved without the radical (and irreversible) treatment of an implant. Remember, an implant is not a cure. Just a treatment. The success stories here make it seem as if sex after implant is a panacea. For those unable to be satisfied without coitus, it pretty much is, but it is not better than sex was with a fully functioning natural penis and rules out any future treatments or actual cures that may be coming in the future. Unfortunately the future is uncertain and our life on this planet is limited. So, we balance the trade-offs.

When I first joined FrankTalk, I took a couple weekends to read through two and a half years of posts/threads with titles that looked informative. I recommend this to you. It will give you a better basis for forming question and understanding the answers for your decision-making.

Your E.D. history suggests you already have done some research and experimenting with treatments. Sharing that knowledge with us may be helpful to those who follow you. I recommend that to you, too.

I note that Viagra (sildenafil) worked for me for a while and when that became ineffective, Cialis and then Tadalafil (Levitra) each took their turn being effective and then losing power. Have you tried different medications? A switch might give you a reprieve.

To be honest, I really don't know but I would think that after he co-authored that paper that he practices what he preaches. He is a fantastic surgeon, very highly qualified and not only did I not lose length, I significantly gained. Check out this before and after picture... A picture says at least a thousand words. :)

Wes is I believe the only other person (Larry) who did not have a complete corporal dilation during implant. Almost no surgeon tries to fit the cylinders in without first completely destroying the tissue that it goes into, both sides. But yes not doing that is much healthier for your penis beyond any doubt. Do they care? No.

I know eh... I walk around our bedroom naked most of the time and my wife comments and asks why I always stop in front of our full length mirror and stare... she just doesn't get it... I'm sure EVERYBODY on here does. :)
