It's not that simple. There are many BCBS providers, and they are not uniform with respect to coverage. It depends on which carrier you have.
Dave, 80, Maryland - Implant (Titan) 2008 by Dr. Andrew Kramer (failed Sept 2020) - never used due to a stroke that, among other things, ended my sex life.

Best call his office and let them run your numbers. Some policies cover, some don't and some doctors are not contracted to some plans within a same company. BC/BS is chopped up into many companies with the same name. Medicaid will not pay 1 cent toward anything erectile dysfunction related. Don't confuse medicaid with medicare which is the seniors insurance. Very unlikely medicaid will every cover it, even if congress' dicks suddenly and collectively start to fall off. Hope this helps.
LGX 21cm .Milam 01/13/16. Horror; both service and surgical outcome. hated infrapubic installation. Kramer revision 03/01/17. 22cm Titan +1.5cm extender. Those who think their opinion is the only one that matters are a danger to themselves and others.

Yeah the insurance industry is very bizarre and confusing. I'll call Eid's office Monday and ask them which plans they cover.
