Thank you all for the replies.
I really don't want this topic to become a debate about "injection yes/ injection no". I just want to focus on the achievement of a free sexuality as free as possible from side effects.
I know the implant can provide this freedom.

But what scares me is: Implant is just a final step to take after the medical therapy stops working. Why that? Because if something goes wrong (infection, erosion and other complications) a man can always say "it's ok, it went wrong, I had no options anyway before the implant, so I can deal with the surgery gone wrong and (maybe) accept it".

My situation is: Pills work (even if with strong side effects) and I can have sex at least one or two weekends in a mounth (I could not bear the side effects everyday). So, if something goes wrong with the implant surgery, I would never forgive myself because I know pills work. I'm really in a miserable situation, and I don't want to mess it up more to become more miserable than it is.

It's impossible to predict the outcome for anybody - there are countless variables in play. All you can do is select the most experienced and skilled surgeon you can to ensure the likelihood - but never absolute certainty - that you will have a successful outcome.

It's true that there is "no going back" after an implant, but if you are seriously contemplating the surgery then you have already acknowledged that there is nothing to go back to. But be aware that surgeon experience and skill are crucial - you can find plenty of discussions about surgeons here - from the "rock stars" to other less known but still excellent practitioners, as well as less competent doctors who have had less successful results.
Dave, 80, Maryland - Implant (Titan) 2008 by Dr. Andrew Kramer (failed Sept 2020) - never used due to a stroke that, among other things, ended my sex life.

Well, if pills are still working for you, I would keep using those until they don't and keep the implant idea in the back of your mind as a possibility. I'm sure you know to use the lowest possible dose that still works for you, as this can reduce the side effects. I am not sure where you live, but here there are four or five different pills to try in different dosages. It may take some experimenting to find the right pill and the right dose for you. Cialis also comes in a low daily dose formulation which may also work for you. You end up with a steady level of Cialis in your body and are ready whenever the need arises. (if it works for you). It is only 5mg a day as opposed to the on-demand 20mg pill. It didn't work for me but seemed to be successful for younger men who tend to be more active.
