I don't recall who told me this but he was a reputable source. Health insurance companies follow a budget. If their claims are running over budget in a certain month. They'll deny valid claims that are not life threatening. A form of triage so to speak. If you get denied coverage, I think it is wise to recheck all of your paperwork & reapply the next month. Perhaps reapply several months if needed.
Oh, be sure to verify that all the correct medical codes are used.

I would not ever suggest counting on this but want to throw this out. Before having my implant surgery, I received a denial of benefits letter on the implant. I appealed it and was denied again. I went ahead with the surgery because it was really important to me and I had the ability to pay out of pocket.
Much to my surprise when I started to receive bills from the hospital, the insurance company covered their portion of the implant surgery. I do not know if this was a mistake on their part or what but it worked in my favor. I did not call and ask because I didn't want anything to change with them covering it.

For what it's worth there's a website, surgeo.com, that has a number of procedures available as a package purchase including implant surgery available from various practitioners, some of whom are referred to regularly on this forum. Financing is apparently available.

Surgeo is a great option.
There are some physicians that will accept Care Credit (Karpman may be one).
AMS now has financing options through Ally Bank, Clavell and Blick use them.
