All engorgement occurs inside the tunica. There are tissues outside the tunica too but they don't engorge, aside from the spongiosum surrounding the urethra. If the implant cylinder fully expands the tunica, there should be the same amount of girth regardless of arousal level. If the diameter of the cylinders isn't as large as the interior of the tunica (why the hell not? Dr. didn't size correctly?) then there might be additional girth from arousal beyond the cylinders' abilities. That's an unknown variable.
