As I understand it, to position the tubing where they exit the implant. That spot has to be close enough to the incision so the tubing will allow the pump to sit in a good spot.
I gather that the incision is placed before the surgeon knows the depth of the crus, otherwise the positioning might be more precisely predicted BEFORE the cut is made.
