Oct. 21 was my one year anniversary of having a 28cm Titan installed by Dr. Hakky. Sadly I chose the same week to get a Rezum procedure for BPH, so I can't really celebrate in style yet. But here are my thoughts on this procedure and the hardware after living with it a year:
I really, *really* love it. It works great, I can have sex as often, as long, and in any style that I want. I never have to worry about "will it work this time", "am I too tired", etc. It's just there all the time. My girlfriend has never known me without the implant, do for her it's just like I'm the most studly man she's ever met. She knows intellectually it's because of the implant (the ability anyway, the constant desire is all mine!), but she still loves that it's something that never requires us to plan, or think about. It just works.
As for the procedure, it's no joke. In retrospect the recovery is not really that long, but when you're going through it and you're in pain and especially if you have some complications, it feels like forever. The risks of infection, early device failures, etc are very real. Patients should think about it as a major, potentially life-altering surgery. I felt like I had little to lose, so it was the correct choice for me. But I also know I've signed up to a life-long commitment of periodic revision surgeries, and I don't get to pick when they will be required.
As to the hardware, I feel like the Titan gets an undeserved amount of hate. Structurally, the only real weak area is the tubing, which accounts for something like 80% of failures simply because the rest of the implant is so robust. That's not to minimize the issue and I'm glad it's being addressed in the new Prime model. I never had dog ears, nor did I find the flaccid look unnatural, though when actually fully deflated it does look a bit collapsed. But it's kind of hard to get it that way without some effort. Usually it retains just enough fluid to look natural and normal.
At first it it was almost impossible to hide, especially with a 28cm implant. But it has softened up remarkably, so now if people look it still looks like a decent package, but it's not hard to conceal. The pump has also softened up a LOT, at first it was stiff and now I'd say it's actually quite soft and easy to pump. It takes about 22 pumps to rock hard, 30 to superhuman stiffness (where it will get sore pretty quickly) and a few more will give me the flat pump, but I never need to pump that much so it's no issue. Hakky put 130ml in the reservoir and was surprised I maxxed it out in less than a year. He can add more but I see no need for an additional procedure at this time. My girl can't take it that hard and neither can I, lol.
I really think the implant has some downsides, but it's a huge net positive. It gives you the kind of sexual performance that all men really want, and I have started to feel bad for men with implants, even if their equipment works just fine. The implant is just next level performance, and it feels like leapfrogging normal and going right to super power.
