It can certainly be hidden, but how would she feel if she knew you never told her about your implant for decades? Would that strengthen or weaken your relationship? I’d guess the latter.

What’s the point of concealing something so intimate from a spouse? Would you be pissed if she had breast implants or some kind of vaginal implant, and never told you? Ignorance isn’t bliss IMO.
