Original IPP May 2022. 20cm Titan with 1cm RTE-classic pump. Infrapubic
Terrible reservoir placement. Migrated out from under muscles just to left of Penis and directly under belt line. Placement was due to previous double hernia mesh from 28 years previously. Function of IPP has been perfect for 4 years, just install issues that are getting worse.Dr obviously didn’t have experience necessary to place reservoir in this place. Was getting painful and got in my way a lot when leaning over a railing or even bending over. Also had tips of cylinders beginning to erode out the bottom of glans. New Dr felt it could possibly last a few more years but was in danger of erosion. Was told if we were going to fix any issues we would be doing a total replacement.
Revision 24cm Titan with 1cm trimmed off Classic pump scrotal install Revision done on February 25 2026. First 4 weeks were great! Very little pain and almost no swelling. Took maybe 3-4 pain pills during this few weeks. Around the 4th week started noticing some swelling in the old reservoir location,so started two oral antibiotics for 14 days. At 5th week got verbal permission to start cycling. Every time I cycled it would go good until I deflated then I would get a fire burn in glans that wouldn’t go away for 10-12 hours!! Talking a 10 on pain scale!! Started taking pain pills so I could attempt cycling. By the end of first antibiotics I started swelling in scrotum so Dr changed antibiotics and we decided it was time to remove IPP and go back with MPP and revisit in 5-6 months.2 days later swelling went completely away and felt better than it had in weeks. Still had terrible pain in glans after cycling. Dr and I both felt it was from a tunneling procedure he did during the revision. Felt since swelling went away so abruptly that it would be worth staying on these antibiotics and see if they could wipe the infection out completely, so continued another 2 weeks of antibiotics. Pain was rediculous when cycling. I attempted sex at 5 weeks and was a disaster. Attempted sex again at 6 weeks and was able to climax but had to endure 12 hours of excruciating pain afterward. At 8 weeks Dr and I decided it was time to do surgery and remove IPP again. Looking at the next week for surgery but a dear friend of 40 years was given days to live and my wife was the funeral director that would be taking care of family… so we delayed surgery to the 10th week.
Plan was check into hospital 4 days prior to surgery, administer 4 day of IV antibiotics. Have surgery on day 5 and send home with a PICC line placed to have an additional 10 days of IV antibiotics post surgery at home.
Had my salvage surgery on May 13.Removed IPP and found only traces of infection after the 4 days of intense IV antibiotics!! Did a salvage and washed and cleaned out thourally! Decision was made to replace with a new IPP same as I had! Dr and I agreed that the chance of infection was very low due to all the IV antibiotics! I’m on day 3 and feeling good! A little more pain today than last couple but it’s mostly in crux area and in shaft, I think mostly due to the wash out and me keeping it pointed up. Also as usual, third day is usually the worst! Sure the nerve block completely wore off by now.
Plan is to stay on this 10 days of IVs and then wait a week or so and see what happens. Dr said within a week after stopping antibiotics we would know if infection was going to come back. Don’t even want to let my mind go to what happens if infection comes back! Just having faith that it won’t! If all goes well I should be released back to work around the first of July!
Say a prayer—— cross your fingers or whatever you can do to send hopes that this will be the final surgery for many many years!
Will mention that I don’t hold any bad feeling toward my Dr! He has been extremely attentive to me thru out this situation and obviously is very concerned for my well being. Makes me feel like he is as upset about the whole thing as I am and thanks me for trusting him thru these last 11 weeks will Continue to update every few days as this process plays out!

60yo.ED started mid 40s pills failed after 10 years. Injections works w diminishing results.
Implanted 5-22 Titan 20cm with 1cm RTE.
Revision 3-26 Titan 24cm with 1cm trimmed off. Revision due to bad reservoir placement and potential tip erosion.

I’m very sorry to hear about your complications, and I’m wishing you the best possible outcome. I’m also glad your surgeon felt confident enough to proceed with placing another IPP and hopefully spare you from yet another procedure.
I’ve been following your posts leading up to and after revision. In fact, your decision to address the impending distal erosion under the glans motivated me to start consultations for a similar issue of my own.
Three weeks ago, I underwent a similar revision. Mine included the tunneling procedure with distal corporoplasty and a scrotoplasty. In my case, the reservoir was reused. Thankfully, I acted when I did. My surgeon told me we caught the erosion just in time. Had it breached the CC it could have become much worse.
So far, my recovery has actually been easier than my first implant surgery. But I take nothing for granted and know I’m still in the most critical stage of potential infection.
Sometimes we share our experiences here on FT and never really know whether they made a difference for someone else. Your willingness to post about your revision helped me move forward, with full knowledge of the risks. I truly thank you for that.
My surgeon told me the infection risk for a first revision is around 5%. A second revision increases to about 8%, with the risk continuing to rise by roughly 3% with each additional revision. The positive side of that is you still have a 92% chance that this third time will be the charm.

Flounder, wow! You must have been slightly undersized! Gained 3cm on revision! Did you notice the difference of the 24 xl being bigger in diameter? I could see a slight difference just with what I was pumped up after surgery. I was able to cycle for about a week before things went south but was very aware of the girth difference between the 20cm and the 24cm.
Anxiously waiting to see how this one goes! Good luck on your revision and can’t wait to see how both of our 24s compare to previous size.

Thanks for the update Rider. First of all it's great that you could get another IPP immediately. Keeping fingers crossed that everything turns out well. Not sure if IV antibiotics kill all the good bacteria in your gut. I read somewhere in a research article that implant biofilms have both good and bad bacteria and the good ones keep the bad ones in check and create a balanced ecosystem. Maybe you want to check with doc how to build back up the good bacterias. Also for a salvage from an infection standpoint I wonder if a CX with inhibizone has any additional benefits. Did you and doc consider this path?
55 years. Using bimix 0.4 units. Works well but inconsistent and very inconvenient. Seriously considering an implant. 6.4 inches bone pressed length to tip, 5 inches girth base, 4.5 inches girth mid-shaft.
