HBP since my 20s Full ED i was 55 when i received my implant January 17th 2017 sever scare tissue through corpora cavernosa clear to the glands (no blood flow) complete revision new equipment july 10th 2023 AMS CX 21cm very happy

I’m happy to read that things are moving in the right direction for you.
Concerning the pain during penetrative sex: I had that too for the first few months. I actually still sometimes get a little bit of an ache if I’m pumped up very hard and have a long-lasting sex act. When that happens, I try to take a break, deflate half-way to give my tissues some rest. Since you did enjoy penetrating your partner, I don’t think you need to worry about the remaining pain. That will fade with time. So go gently for now, if that’s what feels best. Eventually, you’ll be able to pound with abandon.
Your pumped-up dick looks great. You mention that the shaft narrows towards the glans. Well, dicks come in many different shapes (which contributes to the fascination I feel in exploring them). Your narrowing certainly doesn’t look out of the ordinary to me. The same goes for the curvature you mention. I wouldn’t even have thought about it if you hadn’t pointed it out.
You say that your dick points straight out when you wake up. I don’t think that’s the implant. I think that’s your own tissue. It sounds like regular morning wood to me.
Regarding the training regiment you follow to get your flaccid to hang downwards – sixteen hours in a jockstrap followed by eight hours of rest – I wonder if you might be overdoing it. You’re now three months in. I’m not a physician, so I might be wrong about this, but my understanding is that at this point, the scar tissue has already formed. If that is correct, then there might not be any value in following such a strict training regiment. My flaccid feels comfortable most of the time, but sometimes after being forced down in tight underwear for many hours it does get a little uncomfortable, with the kinks of the cylinders pressing against the tissue in the shaft. When that happens, I rearrange myself and put my dick up and to the side. That way, the cylinders are kept almost straight, and I’m comfortable again. (In some types of pants, this may be too revealing, but I often wear an untucked shirt, which covers it up.)
Of course I don’t know for sure that your training regimen for your flaccid is no longer helpful, but this is what I would suggest: As an experiment to see how comfortable you can get: take one day off from your training, wear whatever underwear is most comfortable and put your dick in whatever position feels best.
