I’ve consulted three of the top high volume surgeons in the city, and they all assured me that I’ll keep whatever sensitivity I had before the surgery.
Do your homework on IPPs and find a surgeon who does these day in and day out.

Second entry in the diary of a young man with a Coloplast Titan penile prosthesis.
Good morning, brothers. Today marks 9 weeks since my surgery for the Coloplast Titan 22 One Touch implant. There haven't been any significant changes since my last entry. I’ve had oral sex again on a few occasions, and while I’m still feeling some slight discomfort, it’s not problematic; overall, I’m happy on that front. I haven't wanted to try penetrative sex yet because I assume it would be very uncomfortable and unpleasant. I’m still not sure how much longer it will take for the discomfort during oral sex to disappear or become mild enough to give me the confidence to penetrate my partner, but I’m really in no rush for that right now—I have my whole life ahead of me to do that.
I’m continuing with the daily inflation and deflation cycles to help the implanted cylinders lose their stiffness. I also walk around the house naked for at least an hour a day to let gravity work so the penis progressively drops into a more natural position, which is what really worries me. Right now, my deflated circumference is 11.9 cm. Circumference with the penis in erection: 13 cm.
I’m not worried about the large bulge between my legs or having gone from a 'grower' to a 'shower'; it’s the intense discomfort caused by the stiffness and the upward angle. It’s extremely uncomfortable to tuck it downward in my underwear; right now, I’m only comfortable if I point it upward or to the right. But this can't go on forever because I feel very exposed at work (as a train mechanic, which involves a lot of physical effort and awkward positions inside the trains) and at the gym, where it looks like I have a constant erection. It’s truly frustrating to see how slow the progress is regarding the 'drop' and the loss of rigidity.
I spoke with the surgical team again for advice, and they recommended using jockstraps—a type of underwear that supports the penis and scrotum separately from the leg. This makes it so that every time you climb stairs, bend over, or move, you don't feel the underwear pulling a rigid penis to one side or feel those 'clicks' and the discomfort of having what feels like a 'broomstick' for a penis. It’s true that I feel slightly less stiffness each week, but it’s been 9 weeks now and it’s becoming exasperating. This type of underwear, the jockstrap, is honestly providing much more comfort; my penis and scrotum are like an 'island,' which greatly reduces this discomfort.
Even so, I still feel the penis is huge and rigid, as if it were constricted in my underwear, constantly wanting to break free. The doctors tell me everything is fine and that it's just a matter of patience for the penis to become more malleable and flexible, although they did warn me that it will never—especially with the Titan—be as comfortable as a natural flaccid penis.
Anyway, brothers, the story continues. I still have a thousand doubts and uncertainties, and while I see many positives, I also see many negatives, so I have mixed feelings. I don't regret getting the implant, but the road is difficult.

This is my exact fear about the Titan. My surgery is next week and I’m pushing my doc hard to go ams cx. I do not want to feel what you are feeling right now. I hope it gets better dude.

Can I ask what your bone pressed length and girth were pre surgery and what they are now?
What is the worry? I lost nothing. If anything I am seeing the tiniest increase at 6 months and still cycling to a dull ache daily.

Ah well it never sticks straight out. It takes a long time to soften but once you are able to truly deflate it all the way it hangs just fine.

Good morning, brothers. I am 9 weeks and 4 days post-op from my Coloplast Titan 22cm penile prosthesis implant surgery. This update will be brief.
Regarding the flaccid state of the penis, I’m still having some issues; I know it’s still early and the penis will likely end up hanging more naturally downward eventually, rather than with the current elevation and rigidity. I also believe that residual inflammation in the penis and the reservoir area is causing a slight auto-inflation, which increases the penis's turgidity even more.
Using a jockstrap (athletic supporter) is what offers me the most comfort and allows the penis to stay in the position I hope it will settle into in the future. I try to wear the jockstrap for 16 hours a day (taking breaks when I feel significant discomfort) and I sleep in regular underwear so the scrotum (and the pump) is supported, with the penis pointing upward so it’s secure and the tissues can rest and recover from pointing downward all day. I am attaching an image showing how bent my penis is in the jockstrap (athletic supporter), as recommended by the doctors. It is really uncomfortable and annoying because it forces a penis that keeps trying to elevate all the time, but currently, it is the only thing that compels the penis to point downwards and allows me some degree of mobility and comfort. Did any of you have the same problem? Having a turgid penis in a state of semi-erection at 9 weeks?
But the big update is that yesterday I had penetrative sex with my partner for the first time since my surgery. At first, I was a bit nervous; I had to use plenty of lubricant and help my partner dilate to ensure a smoother penetration. When I inserted my penis, I felt some slight discomfort in the shaft and at the base, but it was very mild.
Once I was fully inside, I started to enjoy it—I finally had sex again. Since it was the first time since the operation, I didn’t want "rough" sex, but rather something soft and romantic. Honestly, despite the discomfort, it went quite well (I’m used to more vigorous penetration, and although this was gentle, it’s understandable given where I am in my recovery).
I'm very happy about that; the money, risks, and time I’ve dedicated to the prosthesis were worth it. I hope to ramp things up in the future and have more vigorous sex every day without pain. I’ll keep you posted.
