Penoscrotum vs infrapubic
I have been lurking around FT for a while in anticipation of my own surgery in May. I note that there seems to be many more men receiving the penoscrotal surgery than the infrapubic. I also noticed that those with the infrapubic surgery seem to experience less pain and scrotal swelling as well as a quicker return to sex than those who had the penoscrotal approach. My own surgery will be the with the infrapubic approach and my surgeon predicts that I will be having sex in 3 wks. I hope he is right, but other than the usual surgical risks, am I being a bit naive to trust this prediction? If 3 weeks is not out of the question, then why aren't more surgeons using the infrapubic approach? Just asking and not being judgemental here.
74 y/r, ED becoming worse following nerve sparing RPP 2009. Scheduled for surgery early May with Dr Perito.

I had the scrotal surgery and also began after3 weeks.
I would worry about palpable tubing, but I really don't know. I believe the pubic approach is easier for the surgeon since the reservoir would be easier to install.

Scrotal, hor. incision just over 1"
18cm AMS 700 CX, 3.5cm RTE 100cc res
Gleason 6 prostate cancer. Monitoring it for now.
Update: On my last biopsies the cancer wasn't found.

My first implant was 4 years ago and had it infrapubic. I had sex at 3 weeks and was still tender but little problems. My pump was placed perfectly and had little to no scrotal swelling. Pump was placed perfectly did have 3 tubes going around side of penis that you could feel but wasn’t an issue. They were buried deep around the side and were never an issue. The biggest issue was the reservoir placement was terrible!! I had double hernia with mesh 20 years prior to implant asi he put it directly under my belt line just to the left of center. He said it was under fascia but was more so just under the skin. It bulged like half an egg! It only got worse over the years to where it was a constant irritation.
My revision was done by Clavell and was done scrotal. Still had almost no swelling and only one small bruise the size of a quarter on my scrotum. Clavell is a great surgeon and spends a lot of time being sure there’s no bleeders which result in swelling. Pump placement is further back where he says he made a pocket for it to stay in and not get involved with nutz. It’s in a different location and can’t be seen from outside! It was a little more difficult to get a grip on at first but I’ve adjusted to that and have no issue now. Dr placed my reservoir all the way up just above my belt line under the muscles of my abdomen. When I deflate it bulges in toward my inside and doesn’t push out at all! It’s at least 7-8” above the incision he made in scrotum but is still placeed perfectly! There’s only one tube going from scrotum around right side of penis that goes to reservoir! I had to dig a lot with my fingers to find it so it will never be an issue. If it wasn’t a little sore I doth think I could have found it at all! Overall this time has been rougher recovery but think it’s due to some tunneling work he did in my glans to place cylinders properly. I’m 6 weeks out and things are definitely turning for the good! Overall I think the scrotal is a better option in the long run but….. the whole process should be considered a long run project!!!

60yo.ED started mid 40s pills failed after 10 years. Injections works w diminishing results.
Implanted 5-22 Titan 20cm with 1cm RTE.
Revision 3-26 Titan 24cm with 1cm trimmed off. Revision due to bad reservoir placement and potential tip erosion.

I've had both approaches and can give you a direct comparison. With my infrapubic I couldn't walk for three weeks — genuinely couldn't. I'm 3 days post-op from my penoscrotal revision and I feel like I could literally run to work tomorrow. (Not that I'm going to test that, following protocol.) The difference in recovery pain has been night and day for me. Obviously everyone is different, but if minimizing downtime matters to you, my personal experience strongly favors PS.
Pills failed in my 30's. Moved on to Trimix. Had a priapism at age 40 on Tri-Mix. End of Tri-Mix.

Infrapubic is faster recovery. Penoscrotal
Is longer. Revisions will generally always be easier than index case regardless of which route.
Surgeon experience is what matters. Youre going with perito who is like the godfather for infrapubic route. Major upside to scrotal is the ability to aggressively size. If size is a major concern then ps is a slightly better route. In experienced hands, you can probably get similar results. Hakky does both infrapubic and penoscrotal.
I had ps with hakky. I was walking normally once anesthesia wore off. I started cycling at day 8. Pump placement is ideal. It is actually easier to place the pump via scrotal. My pump is well hidden behind the testicles at the bottom of my sack.

Became bionic 3/2026. Hakky - rigicon infla10ax 23 cm (22+1 rte). Chronic ed - tried pills, ved, bimix/trimix. Had mild venous leak on doppler. Took the plunge when ultrasound showed fibrosis and i knew it was only a matter of time before i needed one

I believe there is a slightly higher chance of nerve damage with the infrapubic approach.
67 years old. Long term progressive ED. Pills and injections no longer the solution.

Thanks guys for the prompt response to my post. I'm really looking forward to joining the club. Being able to return to a normal sex life is what excites me the most. I'm already partially bionic already as I've got a fake hip and knee so the surgery doesn't scare me. We have a trip planned to a clothing optional resort about a month later, so the quicker recovery following the PS surgery will be a welcomed advantage especially if I can go without my bathing suit. I will have to leave a patch of skin around the incision unshaved until the scar fades.

I've had both, with the infrapubic coming just 12 days ago. I'm already way further along with recovery than the penoscrotal route. My incisions took forever to heal on the penoscrotal and I just felt it's a location that collects more bacteria and I think higher infection risk than the infrapubic approach. I could always feel the tubing on the underside and the penis was always numb where those tubes came out of the cylinder. I can't feel ANY tubing at all with this new one in the infrapubic approach. I'm a firm believer in the infrapubic way of doing it.
