54y DP at 50, DP surgery at 51. Loss 2.5 cm and DE. sex with the use of a ring. recovered the lost cm with 6M of ved. Titan OTR 18cm on Jan 26, 2026. size before: 14 cm length x 15 cm . Size 1 day post-surgery: 11 cm x 14.5 cm

In my humble opinion.....
Should you get an implant? My answer to you is no, you should not. With your repeated questioning and uncertainty, you should not.
Only after ED has got you to the point that you have had enough of erection failure and you can't take it anymore will you be ready and you will know you need and want an implant. No more of this questioning "is an implant better than a real erection", "should I get an implant", "does everyone have problems after an implant", etc etc.
You should be able to tell after all the time you've been here that most, a very high percentage, are happy with their implant and it essentially cured their ED. But, there are some, a small minority, that have had problems of some sort or another.
Nothing is perfect but how bad is your ED to push you to the point of trying anything and everything to cure it?

Considering this, do you think I should still get an implant?

macoza, the question of whether you should or shouldn’t get an implant is one you have to make yourself. It’s fine asking clarifying questions but you gotta stop passing the decision off to others. That alone is a strong sign you aren’t in the right headspace for this, and need to outline the things that matter most for you.
The implant provides rigid and reliable erections. It comes with risks and adaptation like any medical procedure. There is great data here on the forum about what those risks are. It is a life changing procedure - and I mean that holistically. When I made the choice to proceed with my own implant, I knew in my gut LONG beforehand that it was what needed to be done given my own personal choices and long-term strategy to fight my ED, and I imagine this is the same for many forum members.
Take some time to write down your “must haves”. Your non-negotiables. Then write out the possible risks, the different device options, what works best for your lifestyle, etc. Take an accounting of where your personal threshold is for reliability in your sex life. If you’re meeting that with pills or injections, the implant can wait until later. If you’re not meeting that, take some time to really understand how important it is to you to correct that for yourself.
In my opinion, you will know when it’s time for an implant.

33 yrs old. ED since high school. Pills always hit or miss, mostly ineffective. Finally diagnosed with venous leak in early September 2025.

Viagra was never meant to work the next day. Review the literature. Once upon a time I could get 10 hours but it is only meant to last 4 hours.
Cialis on the other hand can be effective up to 36 hours, and daily dose is effective forever, but does require being taken daily (same as most other medications and supplements)
Of course, that all depends on your own body, and can change over time. For me Viagra side effects became intolerable and Cialis just stopped working.
ED since my 40s, managed by pills.
Increasingly severe BPH, emergency HoLEP prostatectomy Nov 2021.
Fixed the prostate but pills slowly became less and less effective.

The pills are not supposed to work over the course of the following day. They work for a few hours after your first take them. This is well known and it's how the drugs are designed. The only person who can decide if you should get an implant is you. But I also think this is the kind of thing you need absolute certainty on, one way or the other.

Youre right about that. But years ago viagra actually worked 48 hours for me, believe it or not. I miss those days big time. I'll try cialis tomorrow

Hey cicero.. appreciate the info. Actually, viagra used to work for me for 48 hours, even 72 in some cases but this was years ago. Today it lingers for like 3 - 4 hours.
Yes youre right that I have to decide. But I'm in a tough spot between pills working still - albeit not as great as it used to years ago - and possibility of implant which I fear for potential loss of sexual sensation and orgasm.

Okay, here's your framework. It looks like pills are still a good option for you, albeit just not perfect. After that, I would seriously move on to shots. When that fails, move to the implant. You want to be in a position where each step is inevitable. When pills actually stop working, move to shots. If (not when) shots stop working, then consider the implant. Until then, don't keep doing this to yourself. You'll drive yourself crazy.
