This is why making the decision for me to get an implant is extremely difficult. I dont want to end up having no or little sexual sensation. I really dont know what to do and I'm scared. Can you offer some advice please?

I can’t say for sure the % incidence of anything like that - just an anecdotal observation from reading many IPP and MPP journals. Have you tried looking around for research on this online or tried aggregating links to research on it with a tool like ChatGPT?

One of these days I should really try to mine the forum for data across the various IPP and MPP journals and do some trend analysis. FrankTalk is definitely a goldmine of self-reported user data, it would be very interesting to get it all organized in a common format.

I think the main thing though is that any surgery carries with it risks - some more likely to occur than others. Some men have issues with sensation and orgasm post-implant, but MANY of these same men see improvements over time. And then there’s a bunch of guys who don’t have this issue at all. You need to decide for yourself if the risks are acceptable to you weighed against the reward: a bionic dick that allows you to have penetrative sex without fear of losing your erection.
33 yrs old. ED since high school. Pills always hit or miss, mostly ineffective. Finally diagnosed with venous leak in early September 2025.

I can climax if it's done aggressively with a handjob or a pocket pussy. With PIV, I can't.

I've had second opinion consults and was told that sometimes the dorsal nerve or its branches can get injured or stretched during surgery. If it doesn't recover by 24 months, it may be permanent nerve damage.

This should not stop you from getting an implant. This surgery can have a small risk of complications, hopefully around 1%. Do it with the hope that you will be in the majority without issues, but be prepared to face it if you experience a complication.

penoscrotal, you are not loosing your sexual sensation completely but in the beginning and during your recovery you might feel less, due to the reasons I mentioned. I still feel numb some parts of the shaft and my glans is not as sensitive as before but should improve with time, dr. words. sex is great, I. dont remember the last time I had a real boner, just this experience gives you so much pleasure and also for her that you dont mind some discomfort. no pressure that you cant perform, like before I knew I would not last to satisfy her and we went into the devils circle, trying failing, trying failing.....
You have to practice maybe different positions and find out what works best for both of you. I can reach climax but it takes maybe more time. And there is another aspect, I have much more desire for sex since the implant, I could literally fuck every day now. Unfortunately its a little bit to much for her :D
just make sure you get a 1st class high volume surgeon than everything turns out fine.

Also I was having issues with anticlimactic and still occasionally happens so what I’m saying is it’s probably not related to the implant rather age and sensation issues as much related to age as anything else. I personally think I overused my VED for many months prior to implant which led to sensation loss for some time. And then the cycling twice daily for most of a year was also stressful on my penis. After I started only pumping for actual use 2 or occasionally 3 times a week things seemed to settle down over then next few months.
59 years old ED started mid 40s pills failed after 10 years. Injections works but diminishing results with pain. Implanted 5-22 Baylor,Scott,and White Dallas.Dr Michael Wierschem, infrapubic Coloplast with Classic pump 20cm and 1cm RTE. Going strong

I don't know why people keep entertaining his repetitive questions when he won't accept any answers because what he wants is (unrealistic) 100% undeniable proof. All you need to understand, macoza, is this: sensation, getting to orgasm, etc., will be for most people will probably be a little more difficult after an implant. Not impossible. Just more difficult. There are further treatments and personal changes like not masturbating frequently that can make it workable or even better than before. Talk to your implant surgeon about this. He can prescribe medication and also suggest changes to your other prescriptions that are making it more difficult. (I got off 200mg Sertraline every night. That's the pill first given to most with premature ejaculation problems, turns out. He asked my psychiatrist to replace it with Wellbutrin. Psychiatrist asked neurologist if he could. Did lab test and was a positive. Change helped a lot.)

And most importantly, this slight change in sensation/orgasm is merely one reality we have to deal with when an implant becomes necessary because we can't get or hold an erection. It won't become superhuman like you want it to. It has flaws. But if you need it, these factors are worth the change and unavoidable.

Are some chances of loss of sensation and getting to orgasm a better or worse problem than being literally impotent and unable to have sex?

thanks very much brothers for your empathy and advice. Just finished attempt 4 with success. I tried not to pump to the max which resulted in more sensation during PIV. Maybe 70-80% like you suggested it. It was still not easy and needed som doggy pounding but nothing like yesterday and today she just put herself on display for me so I could concentrate on myself.
Lets face it, its not like before when we were healthy without ED, but there is no alternative to it in my opinion.
So let's make the best out of it. Time is on our side.

Those things are common initially but generally resolve over time. An implant is not a quick fix solution. It requires persistence and patience to get the best results, usually over a period of several months at least. If that is not an acceptable path forward then an implant might not be the answer for you.

Initially yes, after was allowed to start cycling and having sex at 6 weeks post op I found that I had significant discomfort during sex and some numbness/loss of sensation on my lower shaft. Numbness wasn't a problem where it really matters though - frenulum, glans etc. As healing and cycling progressed sensation and comfort both improved significantly. That is a common outcome for many implantees.

2. Did having penile shortening impede your sexual sensation and orgasm?

No. Physical size doesn't affect sensation or orgasm in my experience. I was shorter post op but regained my original size over time from cycling and just general healing. Orgasms returned once discomfort diminished.

3. How long did you take to get your length back

Probably around 3 months from surgery.

4. Is penile shortening psychological? Or does it really affect sexual sensation?

I don't know what this question means. Shortening is a physical issue, so it can't be psychological. Physical shortening doesn't affect sexual sensation in my experience.

5. Did you use the penoscrotal or infrapubic surgical approach?

Penoscrotal.

6. Which device do you have?

AMS 700 LGX 21 cm x 12 mm with no RTEs, MS pump.

7. Who was your doctor?

N/A. I'm not in the USA.

In my case, it improved.

I became faster; before, it took me longer to ejaculate.

I'm at the beginning, day 16, I lost 3 cm, but I believe I will recover in a few months.

54y DP at 50, DP surgery at 51. Loss 2.5 cm and DE. sex with the use of a ring. recovered the lost cm with 6M of ved. Titan OTR 18cm on Jan 26, 2026. size before: 14 cm length x 15 cm . Size 1 day post-surgery: 11 cm x 14.5 cm
