Insurance approval, but didn't pay after surgery

Need some help from anyone that has experienced this.
I had a implant done over 2 years ago.
Doctors office submitted procedure to insurance for approval.
They got back approval letter so I was good to go.
Now, 2 years later I have a collection company calls me and said they have an account turned over to them for non payment of $47,000.
Insurance did not pay but 500.00 towards the procedure.
Waiting on call back now from Dr office to let them know what's going on.
I would have never had the surgery if I knew insurance wasn't going to pay, but how can they send approval letter, then not pay.?
Peyronie's and Ed for 2 years.

Lawyer time
Oct 7, 2025 - Coloplast Titan XL - Touch pump - 26cm - 0.5cm RTE with Jonathan Clavell. Still sorting out TRT; plan to try a "modified natty" approach without injections.

had the same thing happen to me but my bill was twice that much i tried everything but failed in my attempts just paid a little every month one day the bill disappeared not sure why not going to ask lol paid on it 6+ years actually got a revision was still paying then poof gone ...... they take whatever you can pay
HBP since my 20s Full ED i was 55 when i received my implant January 17th 2017 sever scare tissue through corpora cavernosa clear to the glands (no blood flow) complete revision new equipment july 10th 2023 AMS CX 21cm very happy

Agree with edjohn, time for a lawyer. Wonder why they waited 2 years to try to collect? Hopefully you or the doctor's office has a copy of the approval letter. If so, it should be an easy case to win.
