WEEK 1 - Update:

We’re at the 1 week mark since surgery. Here’s where I stand:

Pain: this has surprised me a lot. Maybe it was reading too many other malleable journals, or perhaps misinterpreting them. Maybe I’m a huge wuss and my pain threshold is low  But MAN - whatever it is, the pain has been rough. I honestly did a decent enough job first couple days but I realize now, that’s because the nerve blocker Hakky used during surgery was DOING ITS JOB 8-) Now that it’s gone? It’s been pretty rough. The main pains I’ve been dealing with have been 1) burning pain from gravity impacting my swelling / edemas 2) remaining burning at tip of penis when I pee, driven by swelling and likely still irritation from the catheter, 3) painful sensitivity of glans and swollen areas - I’m SO sensitive right now man. And it can be quite rough. 4) pelvic floor guarding - I don’t see a ton about this in other journals, but because my brain is so hyper aware of the new hardware in my body + hyper aware of the pain from surgery, I’m holding quite a bit of tension in my pelvic floor. This can be painful at times - it doesn’t last 24/7, but tends to flare up in response to certain pains, walking, etc. Laying on my back and keeping thighs elevated helps a lot with this though and fortunately, it’s not impacting my ability to shit or urinate.

Painkillers: I had two Oxy left but stopped using them - they were pretty effective for about 2-3 hours at a time, but they also kept raising my heart rate and anxiety and I was not fond of that either. So now just sticking to over the counter meds. These help mildly for now.

Sleep: sleep has gotten WORSE since day 4 or 5 or so. Right around the time that the nerve block wore off  honestly, this has been my least favorite part so far. At night, it feels like an impossible mission to get comfortable. I’m in a constant war of keeping my pelvic floor relaxed, and finding the “perfect” back sleeping position (I’m a side sleeper, btw) all while trying to not move? Or maybe move a little bit, because sometimes it’s the stillness that bothers me? Even if you’ve gotten that right, there’s still just the general aching around swollen areas and incision sites. But let’s just say you’ve gotten that all right - you start to fall asleep… it’s going to end in one of two ways. 1) you fall asleep for maybe 30 or so minutes before some kind of pain wakes you, or 2) you start to drift towards sleep, and then BAM - involuntary pelvic floor spasm because (get this), you’re TOO COMFORTABLE

TLDR on sleep: it’s been a doozy. I can’t WAIT for any of my pain symptoms to start getting better because honestly, even just one or two things improving will go a very long way!

Appearance: my dick looks like it’s been through it. Raw from swelling, some small edema. The scar area on back of penis looks fine. Honestly though - even with everything, it’s not too shabby!

Size: too early man. But again - I was a small guy to start with  Hakky feels strongly that I didn’t lose significant size. I feel like my stretched bone pressed length was maybe 0.4 inches more - and so all in all? The size outcome I think is right on track. And again, for week 1, this has quickly slipped to the back of my “give a shit” list :D I just want to HEAL HEAL HEAL now.

Honestly, although this might not appear like the most positive first week update, my morale isn’t completely awful or anything like that! I’m trying to stay positive - it’s only been a week and I’m very early on my healing journey. I’m hoping that things start to slowly resolve throughout Week 1 into Week 2. Let’s see where it goes!

I can't imagine someone be in a positive morale during a surgery recovery period. The pain change how your brain perceives things etc. The anxiety, the waiting.

So i guess the strategy is to hold on, stay strong and think about the fact that 2 or 3 months from now you will be in a totally different mood, also that with a malleable you can forget about revision till the next 20 years.

It’s hard to “quiet” the brain when you’re still dealing with lots of physical pain AND you’re in the aftermath of a wild life-altering decision  but I do think it’s a really important exercise to try and keep your inner morale as high as possible. And totally agreed - a lot of that is telling myself “think of what life could become in a few months time”.

Day 8 post op - afternoon update:

Last night was wild and in retrospect, kind of hilarious.

I got PERFECTLY comfortable very early, so that I could try and get more sleep. It was working - I had my body positioned perfectly, had taken some Tylenol, and found myself starting to drift. And then? FIRE ALARM. To clarify - a literal hotel fire alarm. Loud. Very loud. I could not BELIEVE it.

So there I was, ass-naked, now HAULING my ass out of the bed while carefully not trying to bash my dick on anything while pulling on socks, underwear, pants (so, so uncomfortable), my jacket….

I’m on the sixth floor and although I’m 99.999% certain this is NOT a real fire, I decide to TAKE THE STAIRS in the EXTREME off-chance that I’m wrong.

I get to the bottom (pain. pain), turn into the lobby. And of course, everyone’s just calmly walking around, not caring. It was of course, a false alarm. So back up the elevator I go

I honestly got back to my room and had to just laugh. It was so ridiculous. You have to be able to see the humor in these things

The good news: I slept a little better last night. Maybe 3-4 hours. This was really important for me.

Today? Pain is still here, but I’m managing. I’m back to work (I work remotely in a tech company), and it’s been fine so far.

New symptom: I have a new symptom that I suspect will be very temporary but still worth noting. I think that with the pelvic floor guarding and tension - which has NOT been constant and very much situational - I’m getting sharp “electric jolt” like pains near my asshole  lovely. These come and go and they came on after a particularly painful shower I had this afternoon where I was just uncomfortable from the swelling pains + guarding a lot as a result. I suspect it will only last a day or two and then go away. I’ve also had these kinds of pains before in the past, and they always seemed to appear kind of randomly, are a pain in the ass for a bit (pun), and then go away for months and months without reoccurring.

I’ll keep everyone updated. - Johnny

Man, that is unlucky. But you made it. You even took the stairs! Not an easy feat given the circumstances. And yes, you have to see the humor in these things.

Your "jolts" are part of recovery. I had them, too. I also get more "jolts" in general than your average bear because I sweat a lot (I run warm) and lose electrolytes faster than most people, I suspect. Make sure you're getting enough salt in your diet. Maybe just pound a Gatorade Zero once a day or something. I'd be curious to know if those jolts lessen as a result. It's a known fact that tissue healing requires more electrolytes than you'd need under normal conditions. Lightly salt your food, and have some Gatorade along with your water.

Thanks Duke! Random question - did you find the tip of your penis was burning for awhile after surgery? I’m peeing fine, but on day 9 post op I’m STILL feeling burning for awhile after peeing on the very tip of my penis. It’s definitely not UTI, but really really annoying.

For context: I’m still very swollen too lol.

Day 9 post op - morning:

Sleep / pain: Last night was initially a rough night for pain and I was worried I was going to have another NIGHTMARE night like I did two nights ago. But somehow, my body eventually decided to ignore the pain and send me to sleep. I probably got 4 hours or so? I think that is a small win. I have no other way to explain it other than “I was struggling to relax, worried I would NEVER fall asleep, and then I woke up an hour and a half later to some new pain” it was very trippy.

Guarding / tension: I think I might just be one of those guys who is ultra sensitive to pain and discomfort, ESPECIALLY with regards to my penis. Each pain really makes me cringe and lock up. I find myself reflexively tensing up my feet a lot when dealing with specific sensations. My pelvic floor still guards although this is actually improving a little bit. Noticeably. I still get guarding in certain contexts but am finding that I’m able to almost always “unclench” and as relax as long as I just pause and be mindful about it, which is a good sign that this is very much just in response to the pain from surgery.

I’m keeping on as best I can! - Johnny
