I’ve been following this site for some time, and it has helped me a lot in making this decision.
I’m Brazilian and I’ve suffered from erectile dysfunction since adolescence. My penis would fill with blood, but it never managed to maintain an erection. One day I tried Viagra and it worked, but I had many side effects. So I switched to tadalafil (Cialis), which worked well for several years. However, over time, it started losing effectiveness and became unpredictable.
A few months ago, I tried Bimix/Trimix injections. Sometimes they work, sometimes they don’t. But the feeling of having to stick a needle into my penis every time is horrible. I often get purple hematomas, there is absolutely no spontaneity, I have to schedule everything, and if I want to have sex again the next morning, I have to inject myself again — and sometimes it simply doesn’t work. I see this as completely unviable long-term for someone my age (36 years old).
I also underwent a penile Doppler ultrasound, and I was diagnosed with venous leak (venogenic erectile dysfunction). I discussed these results during my consultation with Dr. Paulo Egydio here in Brazil.
All these years living with erectile dysfunction have also caused me many psychological problems, especially because I was unable to maintain long-term relationships, which deeply affected my self-esteem, my emotional life, and the way I relate to people.
After many reflections and medical discussions, I decided to choose a malleable penile prosthesis, mainly because of my age. I do not want to go through multiple future revision surgeries, and I also don’t like the idea of having to pump a device every time I want to have an erection, which is why I did not choose an MPP.
At the moment, I am still deciding together with my surgeon between the Rigi10 14 mm and the Tactra 13 mm models. He explained to me that the metal alloys used in these prostheses tend to have excellent long-term durability, but that the Tactra may be slightly more difficult to bend and position compared to the Rigi10.
He also explained that, with the tissue-preservation technique of the corpora cavernosa and the presence of residual blood filling, the expectation is that I should maintain measurements very close to my current girth and length after surgery.
I read the reports from Duke Cicero, which helped me a lot, especially because he chose a malleable prosthesis. I consulted a doctor in Brazil, Dr. Paulo Egydio, who is a world reference in malleable penile prostheses. He uses a tissue-preservation technique for the corpora cavernosa and also performs reconstructive surgery aimed at improving girth and length.
I am inclined to undergo surgery within the next two months, and I will write here sharing my experience. I am very grateful to everyone who posts here, because you truly helped me make this decision. I really can’t see myself having to inject my penis every time I want to have sex, and I am willing to face this surgery.

Hey man! I have a thread going right now - Johnnyborg’s Malleable Journey - where I’m sharing my experience with a Rigi10 11mm implant. I’m 1 week in, recovery has been tough but I’m trying to stay positive.
I’m early in my experience, but I’ll warn you that the thicker rods with malleables are not always better. It’s tempting to want the doctor to put the girthiest rod possible in you, but the Rigi10 and Tactra are VERY rigid (especially at higher rod girths) and some guys (not everybody) have a tough time bending and concealing them. More of an FYI than anything.
Duke’s journal is fantastic - it helped me decide that I wanted to go with a malleable as well. Right now, I’m doing the Rigi10 11mm. Duke has a Genesis 11mm which is also well known for its ability to bend and conceal. My working theory right now is the 11mm rods are the “sweet spot” for girth and bendability.

ED since high school. Pills always hit or miss, mostly ineffective. Finally diagnosed with venous leak in early September 2025.
