Kevin’s Implant failure and revision after around two years
Surgery day (December 17th, 2025)
It’s been a few hours since surgery, and I am staying at the hospital overnight. I am thankful that is the process at UCLA. I have only mild discomfort using only Tylenol for pain relief.
Dr. Jesse Mills, my wonderful surgeon, visited me both before and after surgery, and I will see him again tomorrow before dawn for catheter and drain removal. He reported that the left cylinder broke completely, at the base. He referred to torque being more of an issue, the bigger you are. That failure mode is very rare, since the cylinders are very difficult to damage; it’s almost always the pump, followed by the tubes that fail. (There is even an idiot on the internet who demonstrated smashing beer cans, both by pushing them directly head-on, as well as from the side, ostensively to demonstrate rigidity). Dr. Mills replaced everything except the reservoir, which will help make my recovery more comfortable, and hopefully shorter, since there wasn’t that trauma in my belly. There had always been a bit of an issue with the tube from the left cylinder sticking out some, unlike the others that are barely noticeable, even when you try to feel them. I could have had the tube fixed with very minor surgery six months after the initial implant (almost exactly two years ago). However, it was working so well, and I was having the time of my life, so I did not do that. Perhaps it would have avoided this, but that is merely conjecture, and I try to stick to solid facts.
It is good to know that the pump did not wear out, despite at least 4,000 “uses.” The manufacturer rates the devices at 10,000 uses, which at the rate from the last two years, would mean that it would need replacement in less than five years, so I will reduce the number of times I increase and decrease the amount of rigidity. (Read on for what constitutes “uses.”) Having kept track of the number of unique partners, I know that number is more than 2,500. In addition, there were many repeat partners, sometimes the same day. I can only guess that number, based on my activities, but it conservatively adds up to way over 3,000 instances of penetrative anal sex (which is much more challenging that vaginal sex for the device), PLUS penetrative sex with my husband most every day. Masturbation “counts” too, because it requires cycling the pump. That was reliably every day after I healed, and included bending exercises meant to allow increased size, and using a penis pump, or “VED” at least once every day to work on increasing my size. For the first 3 months, I did intense “physical therapy” at least twice a day in order to regain size that I lost from the surgery (which was as expected).
Dr. Mills gave me the great news that he was able to use a larger implant, given all my use and pumping. The largest cylinders they make are 24cm, so he gave me a 24cm implant, plus a 3cm extension, so I now have a 27cm implant. That’s 10.63 inches, for metric-averse” mericans.” (Keep in mind that part of a penis is internal, and the percentage varies from man-to-man.) I can’t wait to complete physical therapy exercises to see what I end up with. I will post progress reports, with photos where allowed.
One day after surgery - December 18th, 2025
I slept only for only brief periods overnight in the hospital following surgery, which is just like it was for my first implant surgery. It was no big deal, as I had my iPad, EarPods, and plenty to stream, with extremely attentive hospital personnel (but lousy snacks LOL). The rooms are very quiet at the UCLA facility, and can be made completely dark. Unlike following my original surgery, I had only very minor swelling and barely any bruising in the morning. A nurse had removed my catheter at 2 AM, which was uneventful (and never fun). The assistant surgeon came in soon after 6 AM to inspect the wounds and remove the drain, which unlike my previous surgery, hadn’t drained anything. It was time for the unveiling—removing the “mummy wrap.” I was delighted that it appeared that short-term size loss would not be much of an issue this time, unlike the first surgery, which took a few months to regain. When pressed against my belly, my dick extended above my belly button, which I used as a very rough gauge. The doctor insisted on dramatically reducing the degree to which I was inflated, believing it would help me be able to pee. In fact, it doesn’t make any difference, as standing at the sink is the only way that works. As a note to men who have this in the future, the way I found to be able to have a BM is to hold a disposable, sterile pee bag that contains absorbent material good for 800ml over your dick while sitting on the toilet. That solves an otherwise challenging issue.
As for the size and shape of my new(est) dick, I am delighted that so much length is already evident, so I don’t foresee any issue in getting back to bigger than I was quickly. Like after the original surgery, during partial inflation, my dick curves quite a bit to the left, and comes out of my body toward the left. I am a little alarmed that the “snake eyes,” the tiny wounds in the head caused by pulling the cylinders into the penis, are not perfectly centered. My concern is that the slight cant to the left in my glans (head), will be worsened, based on the location of the tiny wounds. Time will tell. I took photos indicating the positions, since the healing is very fast, and the photographic evident will soon disappear. I will post the photos.
No one has definitively told me how long the nerve block given during surgery lasts. It is clearly very effective the day of surgery, and seemed to be into the following morning. Once I got home, however, I was very uncomfortable. Perhaps it was partially from choosing to eat breakfast sitting up, or being in an uncomfortable position during the hour-long drive home, from the nerve block wearing off, or some combination of the above, but I chose to use the prescribed Tylenol with codeine for the next two six-hour periods. It was still quite uncomfortable, so I tried THC gummies, which finally allowed me to rest. By the afternoon, my scrotum was considerably swollen. I used as much ice as possible, following the 20 minutes on, 20 minutes off rule. Frozen peas do work much better than anything else. My husband Paul bought me six bags, so that I could use three at a time, while the others were re-freezing.
I still had only minor bruising, although it did increase throughout the day. It was about 90% less than after the original surgery, after which I had major bruising, including on my thighs. The doctor told me that was due to their close connection (not a direct quote) to the surgical area. There is none of that this time, which is great, as that persisted for weeks following my original surgery.
In summary, the only really obnoxious discomfort was for about twelve hours, mostly the day following surgery. As of the late afternoon, I was no longer in any significant discomfort, perhaps partially from staying in bed all day, which is as advised for at least the first three days. I used THC gummies before bed, and slept better than I have in months.
Two days post-surgery

Two days post-surgery I was able to easily control the discomfort by alternating Tylenol and Advil every six hours, as prescribed. The discomfort feels like it is all around by balls, and is at its peak shortly after removing the frozen peas, which I tried to do as per prescribed—20 minutes on, followed by 20 minutes off. Paul helped me cycle three sets of two bags of frozen peas, which work far better than even crushed ice, since they can better conform to the shape of your body.
It is obvious that the “auto-inflation” that I experienced with my first implant is the norm before the pump “breaks in,” as it is just the same this time. The doctor deflated my implant dramatically when he visited the morning after surgery to remove the drain and check me out. Before long, I was about half-hard again, with instructions not to mess with the pump yet. The semi-inflation does help prevent covering the incision, which is the whole point of keeping your dick pointing up for a while during recovery.
An Important Thing to Know in Preparation
I figured one thing out that ought to become part of the standard instructions from surgeons. Being able to have a B.M. Is quite the challenge when there is no possible way to have your dick aimed in the toilet bowl. Even if it would bend down that far, it would not fit inside our elongated bowls. There is absorbent material inside, you just put your dick in it, between the layers of absorber, and you can then pee at any angle without spilling a drop—even if it’s almost straight-up.
Three days post-surgery
Saturday, December 20th
At three days post-surgery, I am doing really well. The discomfort is minimal, and is similar to the dull ache I always go after using Trimix. I am still alternating Tylenol and Motrin every six hours. I’m in no rush to stop them.
I am clearly doing far better than after my original surgery. I know that by all accounts, a “revision” is way less painful than the first time, plus the reservoir was not changed (although I never felt the original reservoir implant). I don’t know how much of the improvement is due to all the tubes being very well-buried, or the lack of the pump adhering to my scrotum (at least so far). The surgical notes indicate that Dr. Mills created a “flap” under which to bury the tubes. I won’t know how well that worked until the swelling is gone or nearly so, but at least initially, the tubes are undetectable. I have read quite a few stories on FrankTalk.org that indicated seemingly unbelievably fast progress with first-time implants that seem to mirror how I am doing now. Those stories might be edge cases?
I am being as diligent as possible to use frozen peas for icing—cycling 20 minutes on, and then 20 minutes off.
The pee bags continue to really be a salvation for what would be a real issue.
Four Days Post Surgery

I experienced morning wood, which was not painful. I’m still alternating between Advil and Tylenol, but the discomfort is pretty minor. This is easy, this time. My biggest concern is how to hide my hard-on when spending Christmas day with my 97 year-old mother-in-law. Tight underwear is the worst possible solution, as my dick extends several inches above the waistband, which is painful.
I finally figured out that stretch (non-adhesive) bandage rolls allow me to keep it against my belly, and can be adjusted to be barely tight enough to hold. So far, I have only tried 4-inch-wide rolls, but I also found 6-inch-wide bandages which I will test before Christmas day.
Unlike when I first got an implant, there is almost no discomfort anywhere on my dick except right at the base, where it meets the uncomfortable area, all around my balls. However, when there is pressure on my dick, it is extremely sensitive and hurts easily, especially with pressure over a smaller area. Like others have reported, it helps to use a children’s sock (ugh!), which I put over a fresh sterile gauze pad each time.
Five Days Post-surgery
December 22nd, 2025
My discomfort has been non-uniform--a little worse each evening. It has been a little sharper today than yesterday, but nothing awful. I'm continuing to use frozen peas, 20 minutes on, 20 off.
I got the great news from the assistant doctor today that I am cleared to suck dick at our traditional Christmas party on Thursday night. Of course I will need to be "hermetically sealed" below the waist, which will be somewhat of a challenge, in that I am at least 40% inflated.
Eleven days post-surgery
December 28 2025
I have been off of any OTC pain relivers for a few days. I have been very diligent about putting frozen peas on for 20 minutes, and off for 20 minutes. The swelling is certainly reducing, as is the bruising.
Based on my notes from my original implant, during which I asked countless questions of my surgeon and assistant surgeons, I began masturbating very gently yesterday, ten days after surgery. I usually vape weed to sleep well, which always turns me on and gets the natural part of me hard. That had been happening for days, resulting in pre-cum without touching myself at all. Therefore, masturbation required only the very lightest of touches. I do not know why, but my sensitivity is off-the-charts, and like when I was a kid. I hope that continues after I have healed. I can cum as quickly as I want, which will be helpful with future collaborations, if it continues. Unfortunately, after having an unnecessary TURP procedure a few years ago (which was just for peeing too often, and did not solve that), its just a dry cum.
Fifteen days post-surgery
I am still alternating frozen peas on for 20 minutes and off for 20 minutes as much as possible. I kind of feel like the old TV series "Lost," where they had to press a button at frequent intervals, but it seems to be helping.
I am still uncomfortable if my balls have any pressure against them. My dick is coming out of my body severely towards my left, and somewhat twisted in my partially inflated condition. (It is obvious in the photos.) The same thing happened with my first surgery, and therefore I believe the surgeon that when it is inflated, it is straight. If I pull it up against my body it almost completely straightens out. If I let it hand, my entire dick starts to burn a bit. Even using a children's sock is uncomfortable, due to the extreme sensitivity, so wearing any sort of pants is way too uncomfortable. I came up with a great solution for not wearing pants while keeping warm in the dead of winter, which is to wear leggings, which provide warmth without touching any sensitive areas.
Since my penis is so ultra-sensitive, the pressure from the waistband from even very large and loose sweat pants is very uncomfortable, especially because I extend a few inches above the waistband, so that it hits in a very sensitive area. I don't know how anyone could go to work at this point, as I wasn't even prepared to go to a New Year's Day party today.

Congrats on your 2nd surgery and your healing process going well. Also thanks for the extraordinarily detailed report. I did see one of your snake eyes pointing a bit far too the side. Did you ask your doc why that was the case?
Also could you please share your proximal and distal dilation length from both of your surgery reports?
I have also seen photos after your first surgery where your dick was pointing straight up when fully inflated. When I looked at the current pics it looked to be at a 90 degree angle. Is that because you didn't go to full inflation yet?
55 years. Using bimix 0.4 units. Works well but inconsistent and very inconvenient. Seriously considering an implant. 6.4 inches bone pressed length to tip, 5 inches girth base, 4.5 inches girth mid-shaft.

Sorry to hear you had a failure. My doctor told me that in most cases the implant surgeries to replace a failed implant is less painful than the first one. I hope you feel little discomfort.
My implant surgery was in May 2023 and I am very happy with my results. It took around 6-7 months before the pain at the head eased-off to a slight sore spot. It's now completely pain-free when fully pumped.
My wife was worried about the implant surgery, but she loves that it looks natural and stays erect. I sure love it.

Yeah and it only happens to less than 1% of the unlucky ones... that’s what my lord told me. And you’re spot on. With every revision, recovery actually gets easier since the tissue is already blunted and dead. You walk away with a bigger implant and a smaller dick, which is honestly perfect for clitoral engagement. Plus, you might even get some extra cheesy resistant bacteria on the house...Can't waut for my next revision!
