I might be asking a dumb question, but why wouldn’t these manufacturers have a blunt tip on the cylinder and aggressively size to expand?

It's been brought up a lot here and mentioned by some of the top surgeons. I would be shocked if the tips aren't redesigned soon. They aren't devastating, clearly, or we would have more horror stories, but the shape of both ends has already been improved in the past and will be again, I am sure.

It could be that the sharper tips provide better anchoring in the glans. The original tissue is all integrated together and naturally anchors the glans. A blunter tip might slide around inside the tunica and give the glans greater undesired mobility.

Just a guess, there has to be a reason the tips are designed the way they are. Blunt tips would be easier, so it seems there must be a reason for sharper tips.

I don’t think so. The dog ears are a result of the thin material of the Titan cylinders forming a sharp fold/kink at the point of maximum bend. Any length cylinder will form this fold if bent enough when deflated.

I have the largest Titan, 28cm, and I don’t really have dog ears. There are two possible reasons I can see for this. One is my tissues might be thicker/stiffer than some men, which could support the cylinders and resist bending.

The second, and I suspect more likely, is that there is fluid coming back into the cylinders keeping them slightly inflated. My surgeon slightly overfilled the 125ml capacity of the reservoir to make sure I wouldn’t run out of fluid down the road from cycling. I suspect that the reservoir has trouble holding that much fluid, and some is staying in the tubing or leaking past the reservoir lockout valve.

The second makes most sense because slightly inflating the cylinders is the accepted standard “cure” for dog ears. I just have it by default due to fluid capacity. I can create dog ears by bending my penis over sharply, but they are not sharp or painful as they are for some men.

Yeah dog ears are just about how soft the material is and Titans take time to get better.
