Uh, it doesn’t. You have to manually inflate it. It doesn’t happen by accident.

The implant does not fill more during sex but some of us continue to get natural engorgement of our penis when we are turned on. Blood still fills out the tissue around the urethra on the underside of the penis. The glans still gets engorged. That would make it seem like the implant had filled further.
