Flaccid states/dog gear questions

Postby AnotherOldFart » Fri Nov 28, 2025 2:39 pm

Hello friends,
My appointment went well, and I'm cleared to undergo penis erection surgery. I need to optimize my CPAP delivery for safer anesthesia.

Can you guys give me a "for dummies" elaborate rundown on how to decide between AMS vs Coloplast? My doctor is leaving the decision upto me. He says AMS is the best in the flaccid state and not the best for girth. I didn't grasp what he meant. Does it give good erections at all?

Coloplast is unbeatable in the erect state and girth fill according to his PA, however, the dog gears inside cause discomfort/pain for about 1% of their patients according to her and those guys get the AMS next.

Can someone explain what dog gears are? Is it some kind of a micro gear for the pump mechanism? Will Coloplast have a flaccid?

Apologies if this question is too basic for the forum. Just confused. Hardly got a breather during the 15 minute appointment.

You guys are amazing for the wealth of knowledge.

Dog ears: Imagine you take a piece of rubber tubing, and fold it in half. Where the material folds you get corners on the edge, like little triangular ears on a dog. That's what can happen with Coloplast implants.

I think AMS doesn't suffer from this (at least not as much) because they use a multi-layer cylinder material, whereas Coloplast uses a uniform thinner (but some say stronger) material.

I have a Titan and I have been thrilled with it so far, and personally I don't have dog ears unless I really bend hard in a way I never really would, and even then they don't hurt me.

Yeah, in the fully flaccid state it's a little unnatural feeling, in that you can feel the cylinders rolling and wrinkling inside with a bit of a "rubbery" feel if you manipulate your penis. And if you bend your penis sharply, that's when you'll get the "dog ears" on the edges along the bend. I don't think anybody could tell without touching you, it looks normal.

However, if you inflate the implant just slightly those issues pretty much vanish...the cylinders smooth out a lot and the dog ears lessen or go away. Doing that does make you look more full under clothing, so you'd have do decide if that's worth it. But ANY IPP will make you a "shower" because the cylinders don't shrink when flaccid the way natural tissues do.

It is true the Titan is ridiculously hard if you max it out...I don't even think most women would find it comfortable like that, and I think most Titan users stay about 80-90% inflated for most activities. I do think the AMS CX series is pretty close to Titan in that regard.

Good luck, PM me if you want any more details or pictures.

AnotherOldFart, first congrats on making the hard decision and getting cleared for surgery ! With regard to your question, you might find the thread below useful and/or ask some specific questions there.

AMS or Coloplast? Which is best for me?

I have the AMS-LGX. It's quite natural in the flaccid state, and when pumped up, I can't imagine it being any harder. 8-)
I'm quite happy with it, as 99% of the time it's flaccid and lets me live a pretty normal life.
Age 59. AMS 700 LGX 21cm + 2cm RTE.
Peniscrotal, 2/23.
6.25" OEM with Cialis, about 5" post-op. (11/01: Length 6.75" and girth is up to 4.75" mid shaft.) Still growing, albeit slowly. :D
Cycling daily. Hoping to get to 7". ;)

This was a consideration I had when deciding which implant would be best for me.... flaccid comfort. I know there are a number of men on here that proudly proclaim "I didn't get an implant for flaccid!" Well, good for them, but for the rest of us, as BigDave said, our beloved penises will be in a flaccid state for 99% of the rest of our lives. Therefore, for me at least, flaccid comfort was a factor.

I don't feel any rubbing, no squeaking, no dog ears and a completely natural looking and feeling flaccid with my CX. It's never felt anything other than part of me. When deployed, my wife prefers it at 80% inflation (16 of 20 pumps before I get the marble). 17-20 is of no use to me, but may be for others. Rigidity is more than enough. As for girth, the Titan cylinders are larger in diameter, which can result in more girth, but remember, your tunica albuginea will determine your girth and length, NOT the implant. The cylinders can only expand to the size of your anatomy, if you are already very girthy, then maybe the Titan is a better option with the other considerations, but don't expect ANY implant to make you girthier than you were at your peak.

Some men complain of a "surfboard" looking penis after implantation, and I've seen that issue mentioned more on FT with the Titan than with the AMS. The "surfboard" is a result of two cylinders side by side, which doesn't result in a perfectly round penis like you probably used to have, but one that is shaped more like a surfboard.

I'm a grandfather, and I have grandkids climbing all over me all the time, which I LOVE. I don't want to be worried about auto-inflation, or stiff cylinders, or crinkling or dog eari-ng. We spend time together at the beach and I have zero concerns in a bathing suit. (my wife and I also visit nude resorts, and there is absolutely no indication that I have an implant, it looks completely natural. As a matter of fact, I had a conversation with a guy that was touting his implant, he had no idea I had one too, and I never mentioned it. I just told him when the time came I'd look into it.... ;) )
