I have some regrets as to my initial choice of surgeon, whom I'm still upset with, and some worries specific to my personal situation. Shit happens and I think I'm okay but I've been through a wringer.
Middle-aged SGM with lifelong ED. AMS 700 CX 21cm + 3.5cm RTEs implanted January 2025 and explanted due to infection February 2025, with salvage. Revision to Coloplast Titan 24cm + 1cm RTE July 2025.

Courage, give us an update, we know you’ve been through a lot with the initial infection. How are you doing with the reimplantation? Thinking good thoughts.
50. Implanted 5/21/2024 at Kaiser SSF. AMS 700 CX 21cm, 3cm RTE. Penoscrotal. Venous leak my whole life. Pills helped, but hated the side effects; worked less as I aged. Skipped injections. Grateful to bionic brotherhood that helped me make this decision.

I think physically I'll be okay. I'd never had an erection that would stay firm enough for penetration, so it's been a success in that regard--there was a previously undiagnosed venous problem that apparently must've always been there. There is subjectively less sensation, but I'm able to masturbate to orgasm. Looks, I've lost a bit of girth and cosmetically it's a bit different--not mangled or bad but it's been through a lot. The reservoir makes itself known, which I'm going to follow up about. As of fairly recently I can cycle up and more importantly deflate without problem; there's minor discomfort in the scrotum where I squeeze the pump and around the incision.
A lot of my specific worries are internal: a lack of "game" and anxiety about desirability. I am *not going to discuss those* beyond saying I'll get where I want to.

Ok, thanks. I think you have the right attitude. I recall thinking over the first six months or so that I had a lot of the same issues. But they all worked themselves out… cosmetically, my dick got more attractive and longer, the bizarre sensations I would get from sex and masturbation went away.
I’m just glad the infection is behind you. You’ll get there on everything else.

From what I know, Mark1974 and duke_cicero also had the subcoronal approach. Mark1974, if you're reading this, hope you're well, brother.
My surgeon told me that the subcoronal approach has a lower risk of infection than the penoscrotal approach, and that it also carries a lower risk of hematoma compared to the PS. He does all 3 approaches.
I’m not comfortable putting out his name publicly. I’ll just say that if you search his last name you’ll get 3,000+ hits, and many members here consider him one of the top five in the US. I'll also say that he’s been awesome as a person to me.

No, I never regretted getting the implant. The fact that mine broke down after only four and a half years and I’m now waiting to get it replaced has not changed that: I’m happy I did it, and I’m happy that I will soon have a replacement, which hopefully will last a lot longer.
With the risk of stating the obvious: you shouldn’t get an implant before exploring the other options. If the pills work for you, you probably want to stick with those. If the injections work, then, well, that depends on how you feel about the hassle and planning they require. Some have a great sex life on them; for others they are too much of a mood killer. There is a reason so many of those who start the injections give them up after a year or two.
And once nothing else works and you still want to have a hard cock for sex, then the implant can be a blessing.
If I could go back in time and give my younger self advice, I would tell not to wait until age 55, but get it as soon as a combination of pills, injections and a cockring was so ineffectual that sex was more frustrating than fun. For me, that would probably have meant somewhere in my mid-forties.
