This professor claims to perform the surgery under local anaesthesia and seems to be the only one in Italy to do so.

The method of anesthesia would be VERY low on the list of things that would decide my surgeon.
General? Fine.
Regional? Fine.
Local? Fine.
This video isn’t local only. You can see a syringe of Propofol attached to the patients IV in his left arm. (The syringe of white liquid). It’s not uncommon for surgeons here in the USA to do a local penile block injection for post op pain control control regardless of what other method they use (general or regional). I’m not sure how widespread that practice is in Italy.
The criteria I WOULD use are: the quality of the outcomes. Are there lots of happy guys with well functioning penis’ that are at or near their presurgical size? Does they use the product I want? Do they use the surgical approach I prefer (penoscrotal vs infrapubic)? Is it local to me? Or if not, is it worth the extra distance.
I let my surgeon decide what the method of anesthesia would be. (Spinal with sedation)
So if this surgeon does great work. You like their outcomes. You like the product they use and their method of doing it, then go for it. I love my implant. I’m guessing you will too.

Thanks i would like have your idea This doctor cuts and inserts the prosthesis. Does it look different from everyone else or am I wrong?

This is a malleable implant procedure. I’m not very familiar with those. I don’t know enough to have an opinion on the surgeon’s technique.
If a doc gets great results and has done a lot of them, and patients are happy with the end result, then the technique is up to the surgeon.

"Semi-conscious" anesthesia is deep sedation, also known as twilight sleep or monitored anesthesia care (MAC). This state is achieved through a combination of sedatives and pain-blocking medications, typically delivered intravenously (IV), that relax the patient while they remain awake enough to respond, but with little to no memory of the procedure.
