I had my follow-up with Hakky today. I admit I was pretty apprehensive because of this hematoma seems to be resolving very slowly, if at all. All of the swelling, bruising, and loose blood has disappeared, but there's still this hard mass on the right side of the scrotum, that tapers down and wraps around the underside of the pump. The mass doesn't hurt and I can cycle, but still frustrated with the slow progress of it healing. Hakky and I covered a lot of ground today, so in order here's what we discussed:
* Hematoma: He said That it will heal, but might take weeks. The pump is adhered a bit to the hematoma, so we wants me to keep gently pulling down on it to break it away over time and move it a little lower in the scrotum. He thinks (and I agree) that the injury was caused by over-aggressive pulling down on the pump when tore the suspensory muscles of the right testicle, causing a bleed. The testicle is pushed up and somewhat surrounded by the hematoma, but it's not infected or necrotic or it would be insanely sensitive. Hakky called this a "small hematoma"...I'd hate to see a big one! He wants me to alternate ice and heat to help break it up. I asked about taking Bromelain as a supplement to help it heal and he said "yes, great idea." Amazon is delivering it tomorrow. He prescribed Celebrex as an anti-inflammatory to help bring it down and let it heal.
* Asymmetric Inflation: I started a thread by this name to discuss this. When I'm reclined while inflating, sometimes the left cylinder will inflate faster or before the right, then the right "catches up" and both are fine. This doesn't seem to happen when inflating standing up. Hakky said what I suspected...the left probably has a more direct tubing route with fewer bends than the right. So the shorter path causes the left to differentially inflate. Once the pressure in the left gets high enough, the fluid prefers the lower pressure path into the right cylinder. He says it's no big deal, and I agree. Glad to have that confirmed.
* Distal tips position in the glans: My glans sits a little pointed upward since surgery, and the tips go into the glans but seem to be closer to the underside then right in the middle. The right tip reached mid-glans, the left is slightly shorter by a mm or so. I told Hakky that I was concerned that during vigorous sex the head my fold backwards and either cause pain for the woman or start to erode through the thinner skin under the glans. He sad the length difference is tiny and the left corpora is longer but not by enough to use even the smallest RTE -- so adding an RTE to that side would oversize it and make erosion more likely. He doesn't think any of this will cause issues, but once healed and having sex if there is an issue there he can do a small procedure to take up some skin under the glans and get it to sit more center so it can't fold back. I'm satisfied with this answer.
* Reservoir Pain: The area over the reservoir has been sore since surgery...there is a top running from the reservoir space under the skin to the right side base of my penis. It's not visible and if you feel for it it feels like a tendon more than anything else. But it is sore. He said it's because he had to suture it in place to keep it hidden (I'm slender and the big full reservoir kept trying to pop out of the space of Retzius). He said the sutures are probably inflamed a little and the Celebrex prescribed for the hematoma should calm it down.
Hard Knot Under Glans: After surgery I noticed a hard, kind of sharp little knot under the thin skin just to the right of and below the glans, about 2-3mm long. I figured it was a tiny embolized blood vessel. When I went to show it to him, it was already gone, lol. He said it was almost certainly where he'd passed the devices up to punch the needle through to make the "snake eyes" (mine are completely gone already, BTW). So, non-issue there since it's resolved.
Incision: He was thrilled with the incision. It looks great. He even pointed out how great it looked to the guy he was training. I put Neosporin and Silver Sufadiazine on it each morning after I shower, and then re-apply that combo a couple of times a day. He said keep that up, and he's going to remove the sutures in two weeks (which will be right at 5 weeks).
I know its sounds like a lot of issues going on, but it's not really. I'm a little OCD and I bring up every little thing I notice, and Dr. Hakky had great explanations and reasoning for all of it and really put my mind at ease. Overall I'm thrilled so far with the results of the surgery and my healing progress, except for the annoying hematoma. But as long as I can keep cycling, make progress, and get healed and have less pain over time, the prospect of this thing taking a few weeks to fully resolved isn't so bothersome.
Whew, that's it for now, Hakky sees me again in two weeks (the day before Thanksgiving) and the sutures will come out and he'll check on my progress.
