How Long Does Cycling Pain Last?

I have been doing my surgeon-recommended twice daily cycling, shooting for 30min at max inflation each time. Sometimes I make it, sometimes I can’t quite keep it that long.

The pain starts pretty quickly, a little bit right away and then within 5min there is aching in the shaft and a burning/stinging sensation in the glans, as well as a tightness from the proximal tips in the perineum.

My question is: How long does it take before this pain lessens to tolerable levels? I can’t imagine trying to have sex as it is now, but I also realize I am still early in the process. Am I looking at weeks or months? Will it just gradually improve, or is there a “tipping point” when it just doesn’t hurt as much? Also how long before it just feels good to inflate, without pain?

You are still early in your recovery. Not even a full month yet. It does take time. My first sex was at 6 1/2 weeks. It was ok but not totally discomfort free.

Early cycling for me frequently was done after taking some either acetaminophen or ibuprofen. Don't be afraid to use otc pain meds. If I couldn't manage full cycling times? I did multiple shorter times per day. I also had more pain in my perineum than in my shaft.

When discomfort goes away is very gradual. One day you'll just remember that hey doing this used to hurt!

I think everyone's different but i was similiar to you.
Heres what helped:
Try perito THE exercises. This involves bending your penis while inflated for less time.

Also:
I pumped to 4/5th max and would leave that for 10 minutes, add 5 more pumps and hang out like that for 5 minutes or so and deflate a little for a few minutes. Go back up a few pumps for a few, deflate a little for a few.
Eventually that gets easier to hold the more inflated state.

Also: I set my alarm in the morning for 15 or 20 minutes before I have to wake up and I pump up to almost max and try to go back to sleep for another 15 or 20. (This is also not a bad way to wake up once you get used to it!)

Also: advil. Even when most of your pain is gone while not cycling. I took 2 advil 2 or 3 times a day for the first 35 days. I think some if this pain you just have to powrr through while your body's adjusting but its like having the flu: you can take medication (advil) so you feel less of it.

Also: being active helped a lot for me. Im a runner so... running. But I walked a lot before I was able to get back to running and I noticed by dick actually felt better after walking. Like more blood had gone through and done its job. Especially where my coloplast bent and crinkled towards the base (right where my dr had removed scar tissue)

elbowroom my surgeon practice no cycling before 6 weeks. I am 5 days post op and I could not I my wildest dreams imagine to pump.
no wonder you have to take 800 ibuprofen or even stronger.
68 years old, good shape but 10 years struggle with VED, pills, needles.
arterial sclerosis comb.with venous leak.
Rigicon infla 10ax, 22x12, 1cm rte.Dr. Beley, Paris. 6.11.25
before implant: 6 x 4.9 bone pressed, flacid maybe halv.

I think you'd be surprised. Its uncomfortable at first. 1000%. But its not worse than what the first 5 days was like partially inflated and (imo) the sooner you get to the pain the sooner you get through it.

For me I was uncomfortable all.the time w/o meds for the first 3 weeks (cycling or not) but advil made a big difference.

I think for most guys its more painful deflating afterward at first than it is inflating.

I think I started cycling at 4-5 weeks, and I did it almost daily through 6 months post op.

Early on it was really uncomfortable, mostly around the pump, which got really sore from use, and in the shaft, but I saw steady gains in girth and length, so I kept at it.

At around 8 weeks post op, I could pump up until I had a nice erection with no pain, and if inspired would take it to orgasm.( often with the help of my girlfriend!). After that nice reward, I would always pump more to gain further expansion, which would cause some pain and discomfort and I held it for as long as I could knowing it was worth it. As time went buy, there was less and less discomfort each time I pumped it up hard.

Now at seven months post op, I can pump it to as hard as I want, without any pain, and am having regular intercourse with full confidence and to mutual orgasm. What a gift!
67 years old. Long term progressive ED. Pills and injections no longer the solution.
Implanted 4/4/2025. AMS 700CX 21cm with .5 extenders

I am 3.5 weeks post surgery and I am not allowed to start cycling for another 4.5 weeks - my doctor is worried about my pre-existing scars which may hinder recovery.

He inflated me to about 30%. But, I have been on a trans-Europe business trip for the last week (and will continue to do so till January). I find that I need to deflate my implant during the day while wearing a business suit and having multiple meetings. I re-inflate when I get back to the hotel to what I think is 30%.

I am really worried that I will not achieve my full size potential due to this weird program I am on.

The pain is still there, especially when I change the angle of my penis; when I go to the bathroom, when I take off my underwear for bathing and when I have do any other adjustment.

There is little bit of pain when I walk around naked and 30% inflated, but it disappears after a minute or two.

I really wonder what cycling is going to be like.

Huh, I probably should post this to my thread as well.

I know all Dr's are different.
And I followed mines instructions.
But i don't understand this approach.

Not cycling makes everything stiffen up and heal without stretching... only to then start stretching.
At best this means you have to heal twice but I could easily see how this could result in a shorter end result.

I had to take a week off cycling after my 3rd week and starting back up again was painful and I had to regain my elasticity.

If my dr told me to wait 6 weeks i would ask him if he might consider it ok to start sooner if my pain tolerance is high and I want to give it a try.

Obviously im not a dr. Just a patient.

I had my first implant done in July 2026 and just had a revision done 4 months later in November! So this is my experience.

I was told not to start cycling until I saw my doctor after 5 weeks. Even though I had an infra pubic incision I was more than happy to delay the cycling program. Things looked pretty ugly down there in the first few weeks.

When I started cycling after seeing my doctor it was very painful when trying to get maximum expansion of the penis, particularly in the tips of the cylinders in the glands. I also seemed to experience some residual pain at the other end in the perineum.

The instructions I had was to inflate in week one (5 weeks after surgery) once a day for 5 minutes.
Second week do it for 5 minutes twice a day
After that for the next 6 months once a day for 10 minutes.
He said just pump to where I felt uncomfortable level of pain and just hold it there.

I was given the okay to go a little more assertively. Eg keep it pumped up for longer periods of time. I also went way beyond the ‘feeling uncomfortable’ level. I pushed it to extreme pain level. That was no fun!

My experience in the first few weeks or so was that when fully pumped up it was pointing up to 1pm. The issue was that it was excruciating to say try move the penis down to 3pm.
I just couldn’t. That made me feel very concerned.

I resolved this my angling the penis to 3pm when I started pumping and then magically I could move the ‘hard on’ up and around. (Range from 1pm to 4pm). All good.

I did some Perito bending exercises but to be honest I didn’t feel comfortable in doing it. I felt I was perhaps putting undue pressure on the IPP and was thinking the penis was being put in some extreme position. I tended to just play around and bend it and stretch it to simulate what would happen if you were having sex.

I would however go out of my way to crank up the pressure to where I was in considerable if not excruciating pain, and just hold it there for say 10 minutes.

Every thing was gradually getting better. Time is a beautiful thing.

If I was going to have sex my plan was definitely not to make it rock hard. I’d pump up to the point where it adequately hard but importantly I didn’t feel any pain and could leave it inflated for as long as I wanted.

Then at about (12 weeks post operation, 6 weeks cycling ) just when I was beginning to feel normal, I found I would start running out of fluid when pumping up. Bummer!

Long story short I had a very slow leak in one of the tubes that go from the pump to one of the cylinders.

So….. I just had a revision done yesterday (replaced everything except the reservoir). This time the incision was at the scrotum.

Sort of like Groundhog Day. My learnings.

My doctor did day the first time around that there was little benefit to go for extreme pain to get better length/ girth. He said by the end of one month cycling any increase in measurements would be minimal. He was right. I used to take measurements on a weekly basis.

This time round I am not going to be so aggressive in maxing out what I can pump in. I’ll do mini Perito exercises…..maybe. I think it is important to do cycling every day. Stretch things out and just hold it for a while. Again I am just going to follow the instructions for timelines.

I will pump up to where I feel uncomfortable level of pain (no pain , no gain) but will not push through to extreme pain if that makes sense. Then I will gradually just move the penis around and not go to any extremes. At the very end I may just pump in one or two more times and not long after push the deflate button.

I am influenced by the fact that my length was pretty close to pre op length. I am less concerned about what length and girth I will end up with.

I will pretty much keep to the specific instructions the doctor gave (refer above).

For anyone considering an IPP and the process of cycling etc just remember three things. Go to a good high volume surgeon you can trust. Listen to his directions and finally, patience grasshopper, patience. It just takes time.

I will see my doctor in the later part of December and start the cycling process again. This time round I will be more layback about how I do the actual cycling program. I definitely won’t pump up to extreme levels of pain. Uncomfortable pain will be my new threshold.

Not telling anyone what to do or not do. I am just sharing my recent experience and revised approach.

My only fear at the moment is not getting an infection (risks increase a little for revisions :(
