Thank you Principles. That was really helpful. One last question….I noticed at one time you had a total implant length of 26 cm and most recently 23 cm. Did the reduced cylinder length translate over to a noticeably shorter penile length for you when aroused ?

I haven't kept accurate measurements, but visually, when fully aroused/engorged, perhaps 1 cm short, and I have a suspicion that is actually because my current Titan is tied up in all sorts of ways inside the tunica (proximally and distally) due to my previous infections and the proximal/distal perforations I had. I believe that restricts the full potential expansion of my engorged corpora, but this is a tangent of pure speculation. In short, when fully aroused, I notice very little size loss compared to my 25 cm or 26 cm implants. Thus, the ring. Without the ring, without any engorgement, the size is noticeably smaller than with the 26 cm implant.

08/20 Titan 22+3. Post op was rough. Best sex of my life. Tubing failed after 26 months.
11/22 Titan 24+2. Infected from Revision.
01/23 Tactra 23 13mm. Salvage.
08/23 Titan 20+3.
09/25 Titan failed 2 days ago, I suspect tubing. Rigi10 next?

Well that is good to know there is something that will help. I will keep you posted on how things progress. Thanks for all the help. I’m going to have the catheter and drain removed tomorrow.

I forgot to add that he placed the implant through a transverse penoscrotal incision but placed the reservoir through a separate counter incision in the “lower left quadrant posterior to the rectus muscle “

I’m not sure why he had to do that. On my previous 2 surgeries he did everything through a single incision.

I'm so, so sorry. My infection experience was traumatic but not as bad. My original surgeon also did a counter incision for reasons that I have yet to get an adequate explanation. I'll never know but I'll surely always wonder if that's where things went wrong.
Middle-aged SGM with lifelong ED. AMS 700 CX 21cm + 3.5cm RTEs implanted January 2025 and explanted due to infection February 2025, with salvage. Revision to Coloplast Titan 24cm + 1cm RTE July 2025.

Thanks guys. I think the Carrion Cast with calcium sulfate would have worked had I not had the perforation.
Once that occurred that set me back another 3-4 months. At that point all the calcium sulfate had been dissolved for quite some time and I had nothing serving as a space holder. Other than daily VED use there wasn’t anything I could do.

Courage - were you ever without an implant following your infection or were that able to immediately do a salvage procedure

Sorry Courage I just read your signature line and saw that you did. What type of infection did you have ?

Looking back I wish that I would have really tried to convince him to let’s just attempt the salvage with a malleable even if the infection was present in my corpora. Not sure if he would have but I regret not trying to convince him.

They sent me home after my 6 day stay with Cefdinir to be taken for 30 days. I improved greatly but within a few days after my last dose of Cefdinir has began having severe pain in the shaft and head of penis. He got me right in and said we would do a salvage surgery with a malleable implant unless he saw purluent fluid in the shaft. If it was present he would do Carrion cast with calcium sulfate placed in my corpora to act as a space saver then wait 6-12 weeks for infection to clear and the go back in with another IPP.
The fluid was present in both scrotum and shaft so he performed the Carrion cast. Approximately 12 weeks later we went with another Titan. He said that the dilation went well but apparently perforated my urethra.
He told me it was odd because he felt no resistance. He told me that he has had only 3 perforations in his entire career. I don’t know how many implants he has done up to this point but back in 2016 he had done about 1000 implants. He is a reconstructive urologist and his practice is recognized as a coloplast center of
Excellence and I think AMS as well. So at that point there was no other option but to allow my body to heal. He speculated that the Calcium sulfate while serving as a space saver may have weakened tissues. He told
Me to utilize the VED once or twice a day
Which I did religiously for many months.
Fast forward to this Friday when we attempted again. I am glad the surgery was successful but incredibly disappointed that he was only able to fit a 18 cm Narrow based Titan. My biggest fear was another perforation or a significant size loss. I was hopeful that we could get at least a 20 or 21 cm implant.
I forgot to add that I am 53 and have had type 1 diabetes for the past 35 years. I am blessed that I have an extremely attractive and supportive wife. I hate this for her as well. Any prayers would be greatly appreciated.

Pills no longer work for me. Trimix helps but injections are tough and painful.

It was mssa, a type of staph infection.
