I will try to make this brief. I originally received a Titan implant in 2016 done by Dr. Jeff Brady in Orlando. It was total of 20 + 3 RTE. He did an amazing job as I had significant Peyronie’s disease before this. I increased length and girth. With time my length in increased to 7 inches plus or minus depending on glans engorgement and diameter about 5-3/4. It was also straight as an arrow. This implant lasted until 2024 when the tubing got a leak.
So in November of last year (2024) I had my first revision with Dr Brady and this time he used 22 + 1 Titan again. Perfect placement of implant and pump. At 7 days post op I developed a high fever and my wife took me back to the hospital where Dr. Brady meet me. My WBC count was off the charts. He started IV antibiotics and keep me there for about 6 days. The culture came back as was identified as E. Coli that was resistant to the Bactrim he had me on following my November revision. They sent me home after my 6 day stay with Cefdinir to be taken for 30 days. I improved greatly but within a few days after my last dose of Cefdinir has began having severe pain in the shaft and head of penis. He got me right in and said we would do a salvage surgery with a malleable implant unless he saw purluent fluid in the shaft. If it was present he would do Carrion cast with calcium sulfate placed in my corpora to act as a space saver then wait 6-12 weeks for infection to clear and the go back in with another IPP.
The fluid was present in both scrotum and shaft so he performed the Carrion cast. Approximately 12 weeks later we went with another Titan. He said that the dilation went well but apparently perforated my urethra.
He told me it was odd because he felt no resistance. He told me that he has had only 3 perforations in his entire career. I don’t know how many implants he has done up to this point but back in 2016 he had done about 1000 implants. He is a reconstructive urologist and his practice is recognized as a coloplast center of
Excellence and I think AMS as well. So at that point there was no other option but to allow my body to heal. He speculated that the Calcium sulfate while serving as a space saver may have weakened tissues. He told
Me to utilize the VED once or twice a day
Which I did religiously for many months.
Fast forward to this Friday when we attempted again. I am glad the surgery was successful but incredibly disappointed that he was only able to fit a 18 cm Narrow based Titan. My biggest fear was another perforation or a significant size loss. I was hopeful that we could get at least a 20 or 21 cm implant.
I forgot to add that I am 53 and have had type 1 diabetes for the past 35 years. I am blessed that I have an extremely attractive and supportive wife. I hate this for her as well. Any prayers would be greatly appreciated.

What a roller coaster you’ve been on. I’m sorry you had to endure all this, and I hope as you use the new device you get back to your original size and shape, or at least close. You and your wife are in my thoughts.

Thank you. It has been quite a roller coaster that’s for sure. I remember with my first implant back in 2016 he had said that while infection risk was only 1-2 percent if you happen to be the one it is 100 perfect for you. That definitely describes it well.

Just curious what type of antibiotic protocol did Dr. Hakky implement with you ?

Sulfamethoxazole-TMP oral started the day before surgery and continuing for ten days. On surgery day before starting you get two IV antibiotics. I can’t remember which ones, but Grok says Hakky uses Vancomycin and Cefazolin. And of course the implant is soaked in two antibiotics as well — Rifampin & Gentomycin IIRC.

Beware, all these antibiotics will destroy your gut flora, and your bowel habits will likely be all over the map for several days after surgery. Mine sure were.

I guess you got eight years of good operation from your first implant, before things started going sideways. Do you think some of this was surgeon error, or just bad luck?

That’s sounds similar to Brady’s protocol. Yes it was a wonderful 8 years. I wish I knew. He did tell me that they had seen an uptick in the infection rate and he and some of the other top implanters had as well. They believe it may have been caused by one of the agents that were using to disinfect the surgical area. I can’t recall the name of it at the moment but it was well known. I was able to even pull up some literature that verified what he said. Give me some time and I will think of it.

Wishing you all the best Grad!! After all you have been through, you sure deserve a break.
Brady just did my implant 3 weeks ago and all is good so far.
Keep us posted on your recovery.

I first went to see Dr. Brady in 2014 after having recently developed PD. We first tried a series of Verapamil injections and then finally Xiaflex. None of this were of any help. He then performed my first implant in 2016. He is great at sizing and he is all around a great guy .i was able to regain all of my former size and the some.
