Some guys complain an implant causes guys to have difficulty reaching an orgasm. If I look back at my history, I was having issues before the implant.
My Dr tried about 3 diff meds and Oxytocin. Nothing helped. Was more depressing than not getting hard before the implant.
I was reading about Peptides. I tried PT141.
I have magically for the past 10 days, had an orgasm every day. Oral, anal or by my masturbation.
It's not cheap but is has unlocked my "issue."

soft glans since ever / some ED from youth now fairly serious ED after much use of constriction/clamping to achieve 100% fullness / sorting out TRT and delaying implant plans until all mental, chemical, and physiological possibilities are exhausted

Floridaspeedo, I've had very good experiences with PT-141 and have posted about it in a few threads. Before the surgery I used it on average once a week and had used it for a couple years. I've gotten it from Mint Rx. I also found that THC/CBC(1:1) drinks work well, as do Sativa gummies. I'm about to try PT-141 again.

Couldn’t you just take something that upregulates dopamine? Or what about increasing free and bioavailable T?

Good point F1. I'm currently experimenting with Caborgoline and Clomid to up my dopamine (+effectively keep prolactin to a low normal) and up my endogenous T to a high normal. Caborgoline has a non-zero risk of heart valve fibrosis when taken in high (Parkinson's) doses for sustained periods of time. I want to maintain my fertility at the moment, so not using exogenous T.

I had mixed results with PT-141 (a/k/a Bremelanotide a/k/a Vyleesi) and suspended use once I started TriMix which guaranteed an erection and also seems to help with orgasm.
Born 1954. Diabetes, hypertension and atherosclerosis. Sildenafil is iffy. Tri-Mix (30/3/20 Pap/Phen/PGE1) a godsend pending long-term efficacy. Daily Cialis. Tried LiESWT, Botox, PT-141, Eroxon, QST, DUS, Vertica, cabergoline, psychotherapy+hypnotherapy.

Caborgoline reduces prolactin (targeting low normal levels), is used off-label for DE, though the evidence base is not robust. They say it's a long-acting dopamine receptor agonist. It's not without some crappy GI side effects. No free lunch I guess!
