First post/Residual Erection After Implant

Long Time Lurker, First Post – ED Journey & Consultation with Dr. Eid

Hello to everyone in the FT Community! I've been lurking for quite some time and finally decided to make my first post. I’ve gained a lot of insight from reading through the different topics and discussions, so thank you to everyone who’s shared their experiences and knowledge here.

A little about me: I’ve been struggling with ED for the past 21 years. The only way I can get an erection is with a tight cock ring, which gives me about 95% rigidity. The issue, though, is that as I get older, it’s becoming harder to maintain those longer sessions or enjoy certain positions with my wife.

Over the years, I’ve been leaning more toward the idea of becoming “bionic,” and I’ve scheduled an in-person consultation with Dr. Eid this September. While I’m hopeful, I still have some common concerns, especially regarding potential size loss—both length and girth—as well as the use of RTEs. I’ve also heard about issues like reduced axial rigidity or the pump sitting too high on the shaft, which is a bit concerning.

Apologies for the long introduction, but I wanted to provide some context.

My main question is this: What factors determine if someone will experience, for lack of better description, residual erections after getting the implant? Specifically, I’m curious about whether the head can still get engorged. Does your manhood ipartially trying to rise in the morning or during sex. I’ve seen mixed responses online, so any insights or personal experiences would be greatly appreciated!

Thanks again for all the support and knowledge here. I’m looking forward to hearing from those who’ve been through similar experiences!

Contemplative,

if the implant is performed by an experienced surgeon, there should be engorgement in the glans, cause the vessels and nerves to the glans are not damaged.
The same with the 3rd Corpus cavernosum urethra, which stays intact.
That said, one gets aroused and have engorgement, which is the case in my and many others dick, I get aroused, if I'm sexually stimulated, my glans is getting full of blood, I have a great sensation and together with the hard dick, the sex is the best in my life.

Welcome, and thanks for posting.

I was implanted by Dr Eid about 3 months ago and have had a great experience throughout. I have engorgement when aroused and still get spontaneous erections around/on top of my implant. I still wake with morning wood.

I believe the two main factors are the skill of the surgeon, and your natural capacity for erections.

If your are in the NYC area and would like to talk further or have a show and tell, send me a PM.

Thanks @dan_bionic for your response. The graphic helps. Although i must have seen it a million times it just resonated, witj your explantion, that the uretha travels thru a "3rd" Corpus cavernosum that becomes engorged. Corpus cavernosum minor I guess. Lol. This bit of information helps. I really don't want to lose any girth. But the idea of "the sex is the best in my life".
Is very tempting in exchange for some girth.

@GoodWood thanks for your input and personal experiences as I navigate this journey. It is my hope to have the same results if I finally do go through with it. I should be able to make a better decision once I meet with Dr. Eid. Also, thanks for your detailed journey.

Welcome Contemplative. There are so many variables that I don't think a reliable estimate of results can be given, as everyone's anatomy and experience will be somewhat different, but on average most of us seem to be very happy with our implants.

Initially I noticed some size loss, but since starting cycling at 6 weeks I've gradually regained almost all of my original erect length and girth. I have also become a "shower" with a much larger flaccid than before, which seems to be a common outcome. I didn't need RTEs. It was probably just coincidental that my combined proximal and distal measurements matched a standard cylinder length, which my surgeon determined was the correct fit for me.

I'm happy with my pump placement. I had discussed it beforehand with my surgeon, who asked which was my dominant hand (right), and said he would put it on the right. That works for me as I also used to hang lower on the left, so I now look more even. The pump doesn't create much of a visual bulge but you can (and should) feel it clearly so you can operate it correctly. I've had no problems with migration, or position of the bulb or deflate mechanism.

My implant is absolutely rock hard and extremely firmly anchored at the base when fully pumped up. Some say that the AMS LGX isn't as firm as some other types but I can't imagine possibly needing anything harder. I sometimes find it irresistible to over pump it for PIV sex, but then have to back it off or it becomes too uncomfortable.

After my prostatectomy in 2022 I no longer had any type of natural erection whatsoever, only from injections or VED use. But after the implant I do now sometimes get some spontaneous engorgement/fullness as if an erection is about to happen. It isn't an erection as such, but has that very pleasant sensation like I used to get when I had natural erections. It can happen in the morning but also at other times. Post implant I have good sensation where it matters and I can orgasm from PIV sex and other stimulation. I do have some residual numbness on the underside of my shaft just above the penoscrotal incision, which is improving but very slowly.

My understanding is that the glans has a separate blood flow than the cavernosal region, which will have two fairly large cylinders shoved in there. In my own case the glans only becomes firm/engorged when extremely aroused close to the point of orgasm. After nearly 16 weeks post op I no longer have "floppy glans" when erect, I think because the cylinder tips are now very far into the glans from regular cycling. But the glans does remain fairly "soft" even when pumped to the absolute maximum unless I'm very aroused.

Best of luck with your decision, I recommend making a list of all the questions you want answered before your appointment. That's what I did.
I'm not in the US but I'm aware that Dr Eid is considered to be one of the best.
70 y.o. married from Sydney Oz. PC and nerve sparing RRP 2022, but still profound ED since. Tried pills, injections, shockwave therapy, VED. Finally implanted Mar 6 2025 AMS 700 LGX 21cm x 12mm, no RTEs, MS pump, Penoscrotal.
Recovery going well.

You're welcome to ask any questions, that will help you to decide and really believe me, the surgery and recovery is nothing when it's to your sexual life after.
And Dr. Eid is probably a good choice, go for it!
All the best

Like others I do get some noticeable engorgement when stimulated and also occasionally at night will wake me up a little chubby. I use 5mg Cialis on days I plan on having sex and it definitely helps with the engorgement. Also notice I get much more night time chubby for several days after using cialis. As far as diameter goes I’m about a 1/4” larger at the base than I can ever remember prior to implant! And about the same if not a little larger the full length. Engorgement plays a big part in this. When pumping for exercise the first many months it would give me the oval shape really bad as it was very uncomfortable and definitely had no stimulation. By the time I started using it the engorgement started coming back and all is well. Best decision I’ve made in many years to get implanted. Been just over 3 years and going strong!!
59 years old ED started mid 40s pills failed after 10 years. Injections works but diminishing results with pain. Implanted 5-22 Baylor,Scott, and White Dallas.Dr Michael Wierschem, infrapubic Coloplast with Classic pump 20cm and 1cm RTE. Going strong

Contemplative, congrats on your decision to go for it, and best wishes on your journey ! In case you didn't see these, below are a couple of threads with info that may be helpful to you in your consultation(s). Regarding your main question, I don't know what factors determine that. I suppose if a guy has good blood flow and engorgement with those tissues before surgery, hopefully he'd continue to have that after surgery. It would be interesting to see a poll of guys with good engorgement before surgery -- to see what percent kept it vs lost it.
