My Dr. recommended I do the VED (Vacumm Erection Device) for 6 weeks prior to surgery, but did not mention the workout/Intervals.
What worked for you?
2 minutes on, 2 minutes off, 5 reps?

My surgeon was supportive of me using a VED pre surgery, but didn't have any specific advice.
VEDs are not his thing though. I had also proposed trying one after it became apparent that my post RRP ED wasn't improving naturally and pills, injections etc were not working. He said that he had stopped recommending them for ED as a lot of patients complained that they were too uncomfortable.
So I made up my own pre op VED regime based on my own research and experience. I had already been using the VED several times a week for sex and penile rehab for about 18 months. Then for roughly 6 weeks prior to my implant surgery I stepped it up to 2 to 3 times a day for 20-30 minutes each session. I was very keen to regain pre RRP size and would gradually inflate to the maximum level tolerable, and what I thought sensible. In hindsight that was probably too aggressive, but seems to have helped in achieving the results I wanted. After about 9 weeks of cycling I'm pretty much back to my original size pre-RRP. Surgeon seemed to think it may have helped allow him to implant the largest cylinders that my anatomy could accommodate.
I was using mostly a medical grade Vacurect VED, but sometimes alternated with a sex toy version. Neither device has pressure gauges.
I hesitate to recommend my approach as I think it's quite extreme, just sharing what I did.

70 y.o. married from Sydney Oz. PC and nerve sparing RRP 2022, but still profound ED since. Tried pills, injections, shockwave therapy, VED. Finally implanted Mar 6 2025 AMS 700 LGX 21cm x 12mm, no RTEs, MS pump, Penoscrotal.

I have written a lot about this. Please remember I am no expert but my research (i.e. reading stuff urologists wrote) and my reasoning on why VEDs are safe and effective is solid.
One thing I would change from my earlier writings is my first time around I VED'd too much, like hours a day, cause I'm like that. But it's probably excessive and might lead to problems when ~30 minutes once or twice a day is surely enough to reach your goals.

Here's a comment in that post on page 3 about how I did not know about VEDs pre-surgery but after learning them exactly how I used them, and used the Bathmate. It also mentions my first urologist that implanted me was old guard and retiring soon. His VED advice was, "they won't do you any good." He missed that they won't do you any bad either.

The Bathmate is believed to be an overpriced gimmick by the geniuses of the PE (penis enlargement) cult online. They might be right. But this time around I bought a new one and believe there's benefits to simply keeping it in the shower and using for 5-10 minutes however many times you shower. A cheaper alternative is another good option.

I was hoping for an answer like # of reps or intervals, like at a gym
ie: Qty:5 sets of 2 minute holds, with 1 minute breaks between sets.
But it appears to be more about time under tension. 10-15 minutes of constant pressure in a vacuum.
Does that all sound right?

Time under tension is great advice for exercises, I believe. I learned that from a very helpful woman at Supplement Superstore. I don't do sets or reps anymore. On all the machines I push out slowly some and aggressively some and bring back even slower, and I do that for 1 or 2 minutes on each. Think that's called explosive concentric, slow eccentric training.
Someone please correct me if I'm wrong but I do believe that's superior to other strategies.
I think one could reason that the same would be helpful for VED use since it is truly nothing more than exercising a muscle (Hakky told me that) that's not as a strong as it used to be.
I like 10-15 minutes of constant pressure more, but I am not a scientist.
