7 inches to 4, and Cialis only recently stopped working? Get a VED and see a urologist, there is something odd going on. BTW Viagra often has better results at high doses (up to 100mg), so that might help.
58yo Coloplast Titan implant scheduled for 10/23/2025 with Dr. Hakky. Pre-op erect measurements:
8.5"L and 6.5"C

Yeah I mean when Ed hit 5 years ago I immediately lost an inch after that I barely got any erection now when I do get semi hard it’s barely 4 inches I feel like it’s worst at this point I don’t even care if I end up with less size I just wanna be able to get and stay hard

drayan123, very sorry to hear about your challenges. Absolutely agree with ElbowRoom you need to consult with a uroolgist and would likely benefit from a VED. Best wishes.

I repeat my previous conclusion...you need to get a VED and use it aggressively daily, and speak to a urologist ASAP. You may not have actually lost length, your penis could just be relaxed and maximally flaccid because it's not getting erections and good blood flow to keep things stretched out. A VED will reveal exactly what your maximal erect size is right now. Using it a couple of times daily will help prevent any further atrophy.
Good luck, I hope everything works out.

Cialis is the first drug of the first line, they give it as a start, and it is the weakest, the strongest is vardenafil and then comes sildenafil, I had gotten to take 30 mg of vardenafil, or 150 mg of sildenafil and it was fine, in hindsight, today I would have liked to continue with those doses and with all its side effects and limitations, instead of taking an implant, that said, maybe it's time to switch to vardenafil and sildenafil, as regards the shortening, surely the problems are others and not the problem of suffering from ED for 5 years
56, DE since 2010, the pills worked at high doses not well, on 01/23/23 titan one touch 22, Dr. Gabriele Antonini, Replacement from titan to cx 21 with ms pump on 04/03/2025, today I am almost 60 years old and have various pathologies, testosterone

Don't get an implant.
Personally I'm now thinking it's much better to give up sex and keep your natural anatomy and be free. I think a lot of cheerleaders on here are in denial about what a horrible situation we have gotten ourselves into.

Born 6/15/74. I have substantial venous leak with fairly severe hour-glassing, but no hard plaques. My urologist is Dr. Laurence Levine who performed a Doppler Ultrasound and diagnosed me with VL in 2020. Surgery scheduled for Coloplast Genesis 5/19/25

Bit negative aren’t you Mark.. people are in this situation through no fault of their own and trying to make the best of it.
28 years old.
Currently trying injections - may be a neurogenic cause of ED.

I'm not saying that the situation is any fault of their own.
I got an implant a month ago and that was through my own fault and I'm having a lot of complications.
I think the freedom from having an implant and avoiding all the potential (dire) complications is the best way forward and I wish I could go back in time, but if you must get an implant I would advise
(a) Get an IPP and use it very conservatively
(b) Get a world class surgeon ( I did and am still having a lot of issues)
(c) Make sure they are using the penoscrotal method to avoid nerve damage
