Inflation-pump is positioned in one-side of my scrotum...is this common?

Hi guys.

I had my penile implant 2 years ago (by Mr Ralph in London).

On the whole, the implant’s been working fine since. But one thing that’s been very noticeable for me is that the inflation-pump is positioned permanently in the RHS of my scrotum (rather than straight down the centre) – as depicted by the 1st photo below (which is a birds-eye-view). I don’t know if this was deliberately positioned like this by the surgeon, or if it is a mistake.

The 2nd photo below is my attempt to diagramize how exactly that pump is positioned in my scrotum - and how it consequently pulls my penis toward the RHS during erections.

So I wanted to ask others on this board how common it is for the pump to be positioned to one side of the scrotum? Do you reckon this was done intentionally by the surgeon, or if it is likely to be a mistake?

I think that it might help on getting answers to put your implant sugery method in your signature. In other words, was the implant incision in your scrotum or in the pubic pad above your penis?

The scrotum has a septum (divider) between the left and right testicle which prevents the cords of the testicles from getting tangled with each other. The pump will be inserted into a pocket that is formed by "blunt dissection" (separating of tissues without cutting), either on the left side or the right.

It is normal for the pump to be on one side, and as a matter of fact I don't know if it's possible for a properly installed pump to be in the middle. It may stretch the septum tissue between the left and right in time and eventually appear to be in the middle but it will always occupy the side in which it was installed and your testicle on that side will have a roommate.

Thank you Waynetho - that's reassuring to know. So does my photo look normal to you?.. in terms of what one might expect for the pump to be bulging out to one side of the scrotum?

Yours look like it may be pushing your testicle up. Your sac may not have a lot of room in it, and that is a concern about my sac.

Over time, depending on various variables, the pump may center itself like mine is part of the time. Between going nude around the house and recently while wearing my kilts, and having somewhat loose tubing to the pump that causes mine to hang a bit low, it seems to be in the center whenever my scrotum is slack. If my scrotum contracts though, it crowds my left nut something terribly.

My pump bulb is off to the left. I asked my surgeon if he placed it that way because I was left-handed, so it would be easier for me to inflate. He said, "No". The pump winds up where it winds up. Post-op positioning before scar tissue forms can influence the positioning (hence the instruction to "pull down on the pump" in the days after surgery) before everything gets settled in.

My pump has my left testicle sitting atop it. If I want to palpate it, I really have to dig for it.

That's all I know about the implant pump positioning. Not very much.

The most troublesome thing about it is that when I wear form-fitting underwear, the bulge is very noticeable. But it does not inconvenience me.

Huh! My doctor told me the reason my pump was in my left scrotum was because he's right handed and it was more convenient for him.

The bulge you see sticking out in my photo is of the pump - not of the testicle. The pump feels as though it is beneath the testicles.

I also thought the pump might have been positioned to one-side to make it easier to inflate - but that doesn't seem to be the case when I had asked the surgeon. The side it seems to have been chosen to be on seems almost a random decision. But the fact that my reservoir is also on the RHS (i.e. above the penis as where reservoirs are usually placed), my educated-guess would be that the pumps tend to be placed on the same side of your body as the reservoir, to make it surgically easier to place perhaps.

I also read the advice about 'pulling the pump' down in the days after the operation whilst scar tissue is forming - but to be honest my implant was much much too painful during that time to even touch it, let alone trying to pull it downward.

I only had one developed testicle, the left, so my pump was placed on the right side which I really like. It helps fill out my sack and is very ease to operate.
