If the valve doesn't lock into deflate mode (or on some models without a check-valve on deflate), under certain circumstances, pressure applied to the reservoir (by abdominal pressure for instance) can cause the fluid to move back into the cylinders, causing them to inflate automatically.
I believe most pump models available now from both Coloplast and Boston Scientific include a check-valve to prevent auto-inflation in most situations.
