Losing length is not a given. Every man is different, and there's no way to predict any person's outcome. Many lose none, and some gain. This is not a reason not to get an implant if you need one.

I’m 78 and struggled with Ed for the last 25 years with self injections being the only thing that helped me. After all these years I finally after finding this website and reading all the posts elected to finally go through with an implant on June 18th. I too wasn’t endowed either. About the same size as you, but after deciding on surgery both manufacturers of implants suggested I buy a ved from Amazon which I did. I used it about 6 weeks prior to surgery and my surgeon told me that it really helped in increasing my length to a whopping 5”. Now 16 days out from my surgery my flaccid length is by far longer than prior to surgery. I’m not sure what I’ll have when fully inflated in a couple weeks. I feel at least in my case I’ll have more length when fully erect than before surgery. By not having erections for all those years you lose a lot of your length. At least I did. I know it’s a huge decision, but feel in the long run it’s going to help me mentally & physically. It’s been affecting my mental life for awhile so my thought is from all I’ve read that it will help me both mentally & physically in what ever time I have left. Your only 38, and have a long life ahead of you so whatever you decide I wish you success in your decision. Talk to an excellent surgeon about your condition, & continue to talk and get feedback from all the great guys here on Franktalk who are have gone through the same issues your going through. 63 years old. Implanted 12/04/2018 by Dr. Aaron Lentz at Duke Raleigh Hospital with Coloplast Titan 22 cm. ED for over 30 years. Used Viagra and Cialis, later injections, THEN Gainswave: ineffective.

Much good advice I agree with the advice to get a VED to get back any length lost.
Don't assume you'll lose any length from the implant.

18cm AMS 700 CX, 3.5cm RTE 100cc res
Gleason 6 prostate cancer. Monitoring it for now.
Update: On my last biopsies the cancer wasn't found.

Rather have a rock hard small one than nothing i say. You will be able to enjoy long sessions without worrying about going soft and your partner will really enjoy more than just 4 play ( some guys are done at one play)- you can have many plays before fucking her.

Size loss is not guaranteed.
Pre-op vacuum therapy can restore SOME lost length and girth, particularly if recently lost. There are two popular protocols. One involves staying under vacuum 30 minutes twice a day and the other, applying vacuum for a short time, releasing the vacuum and repeating for 30 to 45 minutes, twice a day. (I am working from memory here and the medical journal papers citing the studies have been posted here a number of times - I will post links in a few days unless someone else beats me to it.)
Do you get nighttime erections or morning erections? This is a good sign for tissue health. Flushing fresh blood through the penile tissues is important. (note that the intermittent vacuum therapy protocol I mentioned does that - not as good ad pure arterial blood, but at least some.)
By all means, get good at oral (and manual) and get good at non-sexual and sexual foreplay. You will find women a LOT more appreciative of a man who is attentive, no matter what the size or rigidity of his penis. Women make love to the MAN, not the penis. Be that man and it will not matter to her how well your penis does or does not work. This is truer the older the woman gets. Young women care more about the penis. As they get more experience and wisdom, they care more for the peripherals than the penis.
There was an article in the Spring of 2019 from Hannah Smothers in the magazine, Cosmpolitan you might want to read. It was addressed more towards 20-something women, of course.
30-somethings and 40-somethings are more discerning and are attracted to more sophisticated men who fill not only their sexual needs, but their emotional needs as well. This is where a man who has addressed his problems (including E.D.) head-on and solved them shines. Women are incredibly understanding and accepting if they feel trusted and respected. A man who knows who he is will get unbelievable amounts of support from a woman who herself feels cared for and supported.
By all means, talk to a urologist who specializes in sexual function. Investigate W.E.D. (vacuum erection devices), erectile oral medications (which may or may not work for severe Venous Leakage), injections or urethral suppositories. Implantation is an extreme step and irreversible. But it is the solution that worked for me after trying most of the others.

None of this is easy my friend ... but you have a very high chance of getting all of this fixed in a very satisfactory way if you choose the right doc. First - size loss is very unusual if you choose the right doc (can't say that phrase often enough ... choose the right doc!). If you spend some time on the forum you will see the same names come up repeatedly with positive comments. Second, while you are busy making your plan - buy yourself a Penismaster Pro stretching device and put it to work. There are peer reviewed scientific studies that show it works. It's a bit of a PITA - but you can probably get back to your original 5 inches before you even go in for any kind of surgery. Third, look into the AMS LGX implant and ask your surgeon about it. It is designed to restore/extend lenth at least a little bit. Fourth, I can tell you from experience that 4-5 inches are all you really need to get the job done. As you may know, most of a woman's sensation is in the upper part of her vagina, nearer to the opening. I had no trouble giving my last girlfriend an orgasm almost every time with my little shortie if you know how to use it
Trust me - that's what she is looking for. Finally ... no matter what else you do, do your homework and choose the right doc to work with. Chin up and start problem solving ...

I agree that getting an implant does not mean that you will automatically have a shorter penis. Mine is back to its original size (5" hard; went from the original 2" to the implanted size of 3.5" soft, changing me from a grower to a show-er).
