I had my test shot yesterday with the Uro. They use alprostadil in powder form for the test. Seemed to work fairly well at the low test dose with some minor aching the only side effect. I have basically been given a choice between alprostadil and Trimix. The doctor would not give me a strong recommendation between the two and left it up to me. He’s been very dismissive of my situation (Peyroines with ED). At one point I had to tell him I was well aware he normally treats more serious conditions but I’m still a patient.

Anyway I went with the Trimix which will involve about a 10 day wait. I can always change my mind later. I’d love to get some feedback from the brothers here that have used both medications on the pros and cons ( other than the refrigeration requirement for the Trimix).

I have not used both!

The refrigeration requirements for Trimix are less stringent than reconstituted alprostadil. It's the proprietary forms of alprostadil in the form of Caverject and Edex that don't require refrigeration. Those are expensive.

While neither Trimix or Alprostadil are risk free the Alprostadil is often considered a lower risk for scaring and fibrosis.

If you only got mild ache from the alprostadil test shot I'd think you are a good candidate for Alprostadil shots. Many guys that get some ache from alprostadil find that with continued use the ache moderates or goes away completely.

I'd vote for compounded Alprostadil from a compounding pharmacy. If you can get some of the Caverject or Edex cartridges that has the alprostadil in dry form I'd get some of those too, for emergencies, as they are very shelf stable.

I started with Caverject for a year+ or so with good insurance, then Medicare ruined that. TriMix next (componded) for a couple of hears in varying mixes, and it caused more burn and ache. So after that, back to 80mcg/ML PGE/alprostadil compounded for the last few years. Stil some ache/burn but less. Uro says some people are sensitive to papavarine and/or phentolomine. It took a few years to sort out the optimum with trial and error.
NOT an MD. 71, M51 yrs, CenTX US. Inj since 12/2016, a yr after pills stopped working. Caverject for a yr. 1/2018 Tri-Mix at 30 pap/2 phent/60 pge @0.3ml, now 0.5ml 80mcg/ml PGE1. DE/Anorgasmia setting in since 5/2019, worse now.

My experience with both 10 years ago was that Alprostadil gave me a lot of pain and 3 hour erections, even on small doses.
Tried Trimix which worked well with no pain, but I developed Peyronies (upward Curve of 30 deg mid shaft) after 3 months. I understand from my Urologist that Trimix is well known for causing scarring and fibrosis in some people.
Went back to alprosadil with very carefully managed dose. After 3 years of this I went for an implant and wish I had done so immediately after my prostatectomy
2011 RP, nerve sparing 1 side, Gleeson score 7. Injections with Caverject 5mgm caused severe pain, Trimix resulted in PD. Implant 2014 AMS LGX 18cm +2cm rte. Revision 2021, AMS LGX 18cm + 2cm rte

My first urologist recommended alprotadil and we tested it in his office. Seemed to work well. My first time and I was nervous but it seemed to do the trick. That said, he recommended Trimix instead and told me that the results would be similar but overall he felt it would be easier for me. He retired this spring and when I needed a new strength the new urologist wasn't a fan of alprostadil. She preferred a higher dosage of trimix and we tested that in her office with good results. I kind of still want to try alprostadil so I might bring it up again somedsay and meanwhile I'll watch these posts.
