Am I missing something?

I've tried the shots a dozen times with no results. When the urologist gave me a demonstration shot I felt a numbness and tingle like when I would get an erection.
I've tried from 9-11 and 1-3 and the plunger was always a little hard, indicating I was missing the mark. Do you go straight in toward the center of the penis like I do? Anyway, I think my next step would be to get an implant.

What is your dosage?
Early 30s with ED for years from penis enlargement stretching and jelqing. Implant by Dr Eid on 24 June 2021 with a Titan 24cm with +1cm RTE on one side and -1cm cut off on the other side

Welcome to the forum.
Not really enough information for a good guess but it would still be a guess as I'm not a medical professional of any kind.

Do you know what mix you're using? And how large is your dose by volume in units or fractions of a milliliter. We usually talk in U-100 insulin units. There are 100 units in a milliliter. You may need a much stronger mix. Most doctors start you out with a very weak mix to avoid a priapism. What needle length? You should always start at the recommended injection locations but the range of area you can successfully inject at is really fairly large. If technique is your issue it's more likely a depth issue. Neither too shallow or too deep will work. If the plunger is difficult try adjusting your depth in or out. You don't have do remove the needle and start over to adjust depth. I apparently have narrow cavernosa as my depth range is fairly narrow.

Have you discussed your problems with your doctor?

I'm using alprostadil 1 ml with a 1/2" needle. I usually go in almost all the way and if it is hard to inject I back off little by little. There is no soreness so I guess some of it is going in.

Ok, 1ml of what? How many micrograms per milliliter? I use 16 units (0.16ml) of alprostadil mono-mix at 80 micrograms per milliliter. I would consider a 5/16 needle but not yet. It could be your mix is just too weak. Do you keep it frozen? Where are you sourcing it from? Are you using it in the form of Edex or Caverject cartridges? Edex comes in 1ml doses in either 10mcg, 20mcg and possibly 40mcg of alprostadil. The plungers on their cartridges are really hard to feel resistance with.

I'm using Edex 20mcg that I get from the V.A., it doesn't have to be frozen.

With the Edex cartridges they contain so much rubber stopper material in them that it's near impossible to determine proper placement by plunger resistance. They are nice to have around due to their very stable shelf life.

OK, I'm going to share some off label stuff here that is contrary to label instructions, so don't do this.

My brother also gets Edex from the VA. He gets the Edex 40s. He takes an Edex 40 and mixes it in the Edex cartridge as directed. He then recaps the needle and removes the needle from the cartridge. He then takes three U-100 insulin syringes, 31 gauge, 5/16 needle, 0.5ml capacity and draws up 3 equal doses from the Edex cartridge through the rubber stopper where he removed the Edex cartridge needle. He could also use 0.3cc/ml capacity syringes but he uses the 0.5 cc/ml syringes. He then uses one of he filled syringes and freezes the other two. He wouldn't have to freeze the other two if he used them that week but he would still need to refrigerate them.

I don't know if that would work for you because you haven't determined your dose. IMHO you should certainly ask for the Edex 40s. You might not get three doses from an Edex 40. You might use the whole 40 for a dose. At this point we just don't know. If you are getting the injection into the correct spot then an Edex 20 sure isn't getting the job done. The stuff I use would be equal to an Edex 80 if they made such things, which they don't. But then I'm only using 0.16ml (16 units) per dose. I do often (bordering on usually) supplement with 50mg pill of sildenafil (Viagra) and a novelty/sex shop cock ring.

Can you share some of your history leading to your ED? How long have you had ED? Sudden onset? Prostatectomy surgery? Onset from diabetes? Gradual age related? Cardiovascular? Venous leak? Injury?

Did you try pills? Did they ever work?

What State or metro area are you in?

I tried Edex, but it didn't work for me.
Did you Uro show you how to inject.
You might want to try Trimix.
It worked great for me for a few years.
Started with Viagra at age 40. Worked good for about 12 years.
Tried Trimix injections, worked great for about 8 years
Now Trimix no longer works.
I got implant on Oct 26th 2022.

I am 77y/o, had ED for 4 months about 25 years ago but the erections did come back.

Did you try pills? Did they ever work?
Tried Viagra, Cialis, and Levitra, which worked for a short time (except Levitra, which never worked). I suspect venous leak because sometimes I can get an erection but it doesn't last.

What State or metro area are you in?
I'm in the Arkansas Ozarks.

I'd go ahead and ask for the Edex 40s. I don't know how difficult it is to get in to see your Uro, but having him do an in-office injection with an Edex 40 would be a good idea.

See how he feels about stacking with Viagra. I think most were against that but that may be changing. It works well with me but my response to Viagra alone was miniscule but I do find it gives a noticeable boost to injections.

Then there's novelty/sex shop cock rings. They can slow blood loss and give the the drugs a bit more time to work. But I hesitate to suggest cock rings at this point as there are so many to choose from and it takes some trial and error to find what works for you. Right now it would be another variable and you don't have the basics figured out yet.
