Just starting out the injections road, had a consultation with the urologist today.

Having read an awful lot on this forum over the past few of weeks I was so much better prepared for that discussion than I would have otherwise been.

Thank you to all of you who have posted in the past and all the useful information provided.

As background, 66 years old I have no prostate issues just ED that has been slowly getting worse over the past 10 years.

Have been on Viagra during that time started out with 50mg tablets which worked well and have worked my way up to 100mg with things getting worse as time went on. Now unable to get a satisfactory erection on 100mg and the side effects were significant and really unpleasant.

So decided to take the plunge and see a urologist today.

Being well prepared really helped, after covering off on my medical history which thankfully is not very complex and a quick examination we talked about the various injection options.

I had already contacted a local compounding pharmacy and got details of their standard mixes and cost so when the urologist started to suggest Caverject ($43 AUD for 2 shots!!!) I was well prepared to talk about Trimix.

I had already done 6 training injections myself with syringe and 29g/13mm needle using IV saline so could discuss my technique etc with him, most of this due to you guys on FrankTalk!!

He was happy to write a prescription out and let me work my way up to find an effective dose. So no test injection with the urologist.

Starting with Alprostadil 10mcg/ml Phentolamine 0.45mg/ml Papervine 12.5mg/ml

As I have been getting partial erections with Viagra and on some occasions “just” good enough for penetration I will be starting with a very conservative 5 units and see what that produces. I have ordered Sudafed tablets with the prescription, he wouldn’t prescribe phenylephrine for emergencies.

I probably won’t take delivery of the prescription for a few days so if anyone has any further advice I would be very happy to hear it.

I used Trimix for 12 yrs., loved the stuff. No side effects from using it that long other then gaining an inch in girth, 6.5+" :o :D . Every now and then I would hit a vain that would cause bruising but that would go away in a day or two. My injections would last about 2 hours, very hard, and then another hour with a chubby.

Sounds like the road well traveled, Viagra, Trimix, possible Implant?
I've been down that road. Viagra got to the point it might work, it might not. Then the headaches the next day. Trimix worked good for maybe 4-5 years, but then the same thing, maybe, maybe not. Doc bumped me up to Quadmix. That worked good for 2 months then it was maybe, maybe not again. After 6 months I gave up injecting. Took me 2 years but finally did the implant surgery at age 68. Wish I would have done it before taking all those drugs and sticking myself with needles.
At least you've practiced injections, have good advice from the urologist. Hope it works well for you!

That's a long thick needle. I use a 31g 8mm insulin syringe, but my penis is thin, and so likewise is the corpus cavernosum, so 8mm is plenty long enough, sometimes too long: I occasionally go completely through the corpus and out the other side! I think the thinner needles intercept less tissue, and maybe cause less scar tissue years down the road.
Age 78 in 2023. On testosterone replacement due to hypothalamus malfunction. (Attention depressed guys: low testosterone is a cause.) Healthy health nut but ED due to getting old. Like to keep enough cardiovascular ability to thrust for 30 min.

Well a bit of a disappointment the pharmacy didnt get my prescription away on Monday so will not receive it until tommorrow Wednesday.
I was certainly anticipating its delivery as was my wife who is really onboard with the project.
Will post again once we have given it a try.

I would be looking for another pharmacy! I live in central Arkansas in a town of roughly 20,000 my local pharm compounds their own, Dr called in first script and within hour I had it in my hand... have refilled twice and picked up 30 min from calling it in
58year old ED started mid 40s pills failed after 10 years. Injections works but diminishing results with pain. Implanted 5-22 Baylor,Scott,and White Dallas.Dr Michael Wierschem, infrapubic Coloplast 20cm and 1cm RTE. Going strong and loving it!

Well my Trimix finally arrived this morning.
All set for a test injection at home this evening my wife will be prepared for action but in the knowledge that it might not work first time in which case we will have fun in other ways.
Going to start with a conservative 5 units and take if from there.

First injection was not enough, started with some stimulation with my wife.
Injected in the righr side, all went well 29g 12.7mm BD insulin syringe inserted to full depth then eased back on the plunger and aspirated a bit of blood.
Injected 5 units nice and slowly no pain at all from the mix.
Started to see results on 5 minutes by 10 minutes with some stimulation from my wife got to about 65-70% just enough for penetration but not good enough to maintain it.

Interesting the instructions from the pharmacy said one shot every 24 hours so have pre filled the next syringe with 8 units and will try again tonight on the left side.

It wasnt a towel rack at all but then again it wasnt a trip to ER!

If 8 units are not enough would a 4 unit step to 12 units be a reasonable increase after another 48 hours, the instructions did ssy 3 shots in a week.
Again many thanks to all who have posted here without all of your collective knowledge last night would have probably been a disaster!

I will post agsin after the 8 unit shot.

If you did that well on 5 units I'll bet 8 units will be the ticket. As for how much to bump after 8 units if it isn't sufficient? Well I think I'd remain conservative. Until you're known to be successfully consistent with your injection technique, I don't think I'd automatically assume a less than ideal result is a result of insufficient dose as it could just be an insufficient dose got to the right spot. With you doing so well at 5 units I think you should be careful and conservative with dose bumps.

You're in the corpus cavernosum if there's no resistance to plunger travel. Depressing the plunger should feel the same to your thumb as if the needle were in air. When I feel resistance, I pull out a little and retry because I sometimes go right through the corpus. If that seems not right I try another spot on the same side, or go to the other side.

Injecting outside the corpus usually gives me an annoying and a somewhat painful bruise, but it disappears in a few days or a week. On rare occasions I've hit the urethra which causes blood to come out the meatus (pee hole), but that seems to not be a problem; it stops soon.

I agree with bdoink; I would go up 2 units, if necessary to go above 8, each time.
