dull needle (2nd injection) - regrets - and questions about priapism ?

Hello to all,

I am using caverject impuls which is pretty similar to an auto-injector.

Yesterday I injected and felt I got not properly in, pulled out and used the same needle for another spot. I dont think about the sterile part, as the skin was cleaned and the tissue itself is sterile but more about the needle being dull and causing more damage to the tissue. Anyone experienced/ done the same ?

2nd question is about priapism - somehow with the last 2 injections I got erections which lasted 3,5 and 4,5 hours.

the one yesterday I had to quit with 2 tablets of phenylepinephrine and an ice pack and was half an hour ago from rushing to the ER.

-> The thing is, my "priapism" is only in the proximal part of the penile shaft, and the penis even hangs down straight. But for about 2cm at the base it is completely stiff and hurts.

Is 4 hours definitely to long ? I adjusted my oral dose of pde5i and alpha blockers (I guess the latter one led to the priapism as I did not take it before) and since then are getting those to long erections

I am a bit scared bc on the injection side on the left the penis feels tighter/not so elastic compared to the right side in the flaccid state which might be an indicator of further fibrosis / inflammation.

I'm not a medical professional of any kind. Anything I post is based on my own experiences, at best and hallucinations or delusions at worst. Internet forum advice is likely worth what you pay for it, or worse. Always ask your doctor and follow his/her advice.

First off, you should definitely consult your doctor on these issues.

I have on occasion, for whatever reason, been unhappy with my first needle insertion and have subsequently removed it and reinserted elsewhere. I don't think this caused any issues. Obviously, the less needle pokes the better. I don't think the reinsertion dulls the needle appreciably but it may lose whatever lubrication comes on the needle on the first insertion. Bottom line is I doubt the second needle insertion would cause appreciably more damage that what and extra poke would normally.

I won't get too worried about a 4 hour erection, especially if it's no longer a full blown (hopefully) erection. If it's partially deflated then there is some blood flow. If at 4 hours you're fully hard then of course take whatever you use to get it down but I'd do that before 4 hours. But I wouldn't get too worked up over the 4 hour mark unless you're several hours away from help.

As for your experience with an erection that has deflated but with a small part staying fully erect, well I have no experience with that. My erects don't always go down totally evenly but I've I've never had a small part stay fully erect.

Although I've been known to do it I think it is generally NOT recommended to take pills with injections. I'd want to get the injections figured out first before adding any pills to the mix. And risk of priapism is the reason for the concern.

I'd like to refresh this topic again. I am very sorry if I am posting reoccuring stuff, but being young and finally having some kind of sex life/ erections is something I am grateful for and do not want to lose bc of another foolish mistake (which led to the erectile dysfunction in the first place)

Today I reduced the dose of the pge1 (because of the priapism experience).
From 10mcg pge1 to 7.5 mcg.

erection was good, went to bed and three hours after the injection it still stood at 1 o clock.

used an ice pack, poped 30mg of pseudoephedrine, took a cold shower, didnt bring much, it went down to 3 o clock. not fully hard but hard enough for penetration.

took another 30mg of pseudoephedrin, started some physical exercise and approx 5 hours after the injection it looks like the picture at the end of the post. I guesss thats alright, it looks flaccid but my penis aches.

Edit: now 5 1/4 hours after the injection it is completely flaccid. I guess I am fine again.

my erections are always warm/hot, but I am always scared I will permanently damage my penis (and I am scared of the ER as well, bc there is no specific urological er and they usually use very small gauges causing potential damage to the tunica albuginea.


Another question - would healty volunteers who get a high dose of pge1 get a priapism as well ?

I am under the feeling that the initial dose of 10mcg might be to strong right now. (hopefully meaning my cavernosal tissue is somehow recovering or I just hit the optimal spot with the lower dose)

Most people that inject much more than the optimal dose for them are at risk for a priapism. Keep reducing until you find the best dose.

I know exactly what your experiencing, same here same side. Get yourself examined if you fear it's fibrosis.

Consultations with several physicians, and some of my own experiences, have given me this information:

1. The human penis is designed to endure a considerable amount of anaerobic time. 4 hours is not a firm number. (No pun intended.) It's a ballpark number and action needs to be considered to arrest priapism as you near that point in time. Most penises can go longer without nerve damage. However, the longer the erection, the higher the risk of nerve damage. For most men, nerve damage can start to occur at the 6 hour mark. If there is nerve damage, it has some potential to heal, given enough time (weeks to months). If the nerve damage is severe, it will likely never heal.

2. Ice is excellent for treating injuries but an erect penis is not an injury. Ice constricts blood vessels and holds the ED injection chemicals in place. Ice helps to prolong an erection.

3. Heat is excellent for dilating blood vessels. Dilation of the blood vessels in your penis will help "rinse" ED injection chemicals from it as heat will help to restore blood flow which is what you need to remove the chemicals and end the erection. To accelerate this process, use a penis pump for just a few minutes in warm-hot water, to further dilate blood vessels and increase blood flow. I use this method with warm-hot water after every love making session with my wife. It works.

4. My urologist, a surgeon, says that when the chemical "papaverine" (in both Bimix and Trimix) is injected, it can cause some light tissue damage to the penis. He went on to say that the major injuries and subsequent scar tissues are caused by the wounds created by the piercing steel needles. These are puncture wounds and they are the worst offenders, no matter how sharp the needles. Of course, dull needles will do even more damage.

5. My urologist's P.A. refers to penile injections as "voodoo". Penile responses to them are inconsistent. They tend to not produce exactly the same results using exactly the same amount of injected chemicals, which makes an erection's firmness and duration unpredictable. After discussing the unpredictability and negative consequences of ED injections, she and my urologist have both recommended an implant. I intend to have one.

6. Bimix and Trimix are similar but Trimix has a 3rd ingredient, prostaglandin, that causes Trimix to produce better results (firmness and duration) than Bimix. Prostaglandin has an unbearable side effect for some men, me included: pain. Pain during an erection, and pain long after the erection is gone. For me, that's a stiff price to pay for a good orgasm, assuming I can orgasm if the pain is intense enough. Not always.

8. I regularly take a Viagra and drink a glass of water containing L-Arginine (creates Nitric Oxide for vascular dilation) about one hour before coitus. This has worked extremely well for me. It has also made injections easier and less traumatic, as my penis is already full and enlarged a bit. My erections are harder and last longer with less injected drugs. Viagra, in particular, seems to enlarge the glans of my penis to its maximum diameter.

Used Cialis (2003-2017), penile injections (2018-2019), 4 implants (2020-2021), now have an AMS 700 installed.

To minimize needle damage, I pull the flaccid penis out to full length, then inject. I think the needle intercepts less tissue when I do this. My pharmacy has new 31 gauge needles now, a little thinner. I've probably injected 150 times now, and have no fibrosis or tissue damage that I know of.
Age 78 in 2023. On testosterone replacement due to hypothalamus malfunction. (Attention depressed guys: low testosterone is a cause.) Healthy health nut but ED due to getting old. Like to keep enough cardiovascular ability to thrust for 30 min.
