Robert, Dr Desiderio Avila would probably work for you.
He also prescribed me with dry mix from Community Clinical Pharmacy in Mesa near Dobson and Southern. Dr Avila has an office near Chandler Blvd and 48th st.
Let me know if you need phone numbers.
When he prescribed tri-mix for me, I could fill it 10 times if that helps.

The Mayo Clinic has a campus in Phoenix and Scottsdale. My appointment was in Rochester. I was (and still am) very impressed!! Good luck!

Robert, did you have any luck scheduling surgery?
Dr Avila just did mine and so far, it seems perfect.
I hope to get activated tomorrow and will know more.

Robert, my only complaint with Dr Avila is his communication skills. That also applies to his office in Phoenix.
If you get to see him say You're Tom Hudson's friend.

You shouldn't be there over an hour or so.

Sorry I just noticed your last post.
