alterr,
Treatment of erectile dysfunction with pycnogenol and L-arginine
http://www.ncbi.nlm.nih.gov/pubmed/12851125
"We conclude that oral administration of L-arginine in combination with Pycnogenol causes a significant improvement in sexual function in men with ED without any side effects."

Acetyl l-carnitine plus propionyl-L-carnitine improve efficacy
http://www.ncbi.nlm.nih.gov/pubmed/16286128

So the best combination would be the above and one of the PDE5 inhibitors ...
Tadalafil = generic Cialis (ignore the "not for consumption in United States")
Sildenafil citrate = generic Viagra
Desylrel = trazodone
Vardenafil = Staxyn
Avanafil = Stendra

Forskolin looks like it is more for weight loss?

I prefer Male Extra. For me it is the best male enhancement pills. You can read review - https://eddrugs2017.com/male-extra/
If you don't like mixes of supplements in male enhancement pills you can try Forskolin.
Forskolin is made from the root of a plant in the mint family. The plant grows in Nepal, India, and Thailand. It has long been used in traditional Ayurvedic medicine. Usually it is used for weight loss but it also boost testosterone levels. So Forskolin is pretty much interesting stuff. Here is good review - https://forskolin.reviews
Also i could recommend testosterone boosters.
Popular testosterone boosters that you may see on health store shelves or read about on the internet include D-aspartic acid (D-AA), tribulus terrestris, fenugreek, ecdysterone and ZMA. In each case, you will find it hard to locate reliable scientific evidence proving that these substances work as testosterone boosters.
And they also can help with erectile dysfunction. Good review - https://naturaltestosteronepills.com

Best thing is cardiovascular exercise. Seriously. Try running every other day for a couple of months.

Huge difference in EQ.

Not sure what "best" means. Most effective? Least side-effects?

I take 8 grams of l-citrulline malate daily. I have not noticed any side effects. Scientific studies rate it less effective for ed than viagra and other ed drugs but still effective for mild cases of ed.
I use a pump for sex and don't need to use a small dose of PDE5 inhibitor to "speed" the time to full erection. We have sex twice a week and after a little over a month on this stuff I'm starting to wonder if I can get her to agree add another day.

I have read where people are starting to use it in place of l-arginine.

The following links explain the supplement.

Before my ED got too severe, I found l-arginine (5 gr) and tongkat ali helped to some extent.
