Prostate cancer codes

I found these code on my prostate cancer 4 years ago can anyone explain what they mean.

3/17/21 Gleason 3+4=7, pT2N0, + right apical margin, negative SV/NODES/EPE

Your pathology report shows favorable intermediate-risk prostate cancer. The confined stage (pT2N0) and absence of seminal vesicle or lymph node involvement indicate the cancer had not spread outside the prostate, though a microscopic positive apical margin means some cells were close to the edge of surgical removal.

Can we assume you had your prostate removed (prostatectomy)? Have you had your PSA checked recently?

Yes! I had my prostate removed in 2020 and 4 years later the cancer returned. In 2025 had radiation and my last two reading were 0.04 and next blood work is due in September.
