BPH treatment

From Nigal Shaw the inventor of Xialla:

Today, I'd like to spotlight the pioneering work of one of our valued partners, Dr. Dean Elterman, an academic urologist at the University Health Network and University of Toronto, who specializes in BPH (Benign Prostatic Hyperplasia), neuromodulation, and functional urology.

Dr. Elterman has been at the forefront of minimally invasive therapies for BPH, particularly with the Optilume BPH Catheter System—a drug-coated balloon that combines mechanical dilation with localized paclitaxel delivery to provide durable symptom relief while preserving sexual function. In recent years, his contributions have included:

- Introducing First-line Interventional Therapy (FIT): In a groundbreaking 2025 publication, Dr. Elterman proposed FIT as a novel paradigm for BPH management, bridging medications and surgery with minimally invasive options that offer rapid recovery, minimal side effects, and preserved treatment flexibility. He emphasized how emerging technologies like prostatic stents and balloons align with this approach, potentially transforming patient care.

- Advancing Optilume BPH Outcomes: Dr. Elterman has shared long-term data from trials like PINNACLE and EVEREST, demonstrating sustained improvements in urinary flow (Qmax) and symptom relief through four years, with preserved erectile and ejaculatory function. In May 2025, he highlighted its efficacy even in larger prostates. He also presented procedural insights at the UCI Men's Health Course in January 2025 and moderated cases at WCET24 in August 2024.

- Conference Leadership and Education: At AUA 2025, Dr. Elterman co-led sessions on MIST (Minimally Invasive Surgical Therapies) for BPH, including data on FIT, Aquablation, and emerging stents like Zenflow. He also contributed to a clinical studies update on prostatic stents published in June 2025, showcasing evolving FIT options. Additionally, he spoke at DSUI On The Beach in January 2025 on tailoring MIST therapies.

- Hands-On Innovation: In November 2024, Dr. Elterman announced implanting every prostatic stent under investigation in Canada, including Butterfly Medical's device, marking a significant journey in FIT development.
