I would sure as heck get a second and third opinion from totally different medical offices before that surgery. Other than cancer I don't understand the necessity.
On the other hand I have a friend that lost both of his nuts in a military skydiving incident many years ago. He's had a very active sex life all these years. He is, of course, on testosterone to compensate for lost hormone production. He also has two silicone nuts in his sack just for cosmetics. He has a wife but no children. If you must go ahead with this procedure you should consider having your sperm frozen.
Edit to add: He was very sexually active with numerous women as a single before getting married.
