Permanent erectile dysfunction caused by stimulants drugs

I’m 29 and have had ED for 11 years. It started when I was 17-18 and using a lot of drugs - marijuana daily, plus occasional multi-day stimulant binges with no sleep (mephedrone, amphetamines, cocaine, or other synthetic drugs). Over time, my erections got worse, and I eventually had to rely on PDE5 inhibitors, which worked great back then.

Now, even though I’ve been off drugs for years, things have only gotten worse. My ED is so bad I can’t have sex anymore, and even during masturbation, I can’t keep an erection for more than a few seconds. I’ve also developed urinary issues like a weak stream, frequent urination, and feeling like my bladder never empties.

I’ve tried almost everything - LSWT, PRP, stem cell therapy, and Bocox—but none of it worked. I’ve seen countless doctors and done tons of tests. Everything came back normal except for my latest Doppler, which showed severely narrowed cavernous arteries with blood flow down to 4-7 cm/s (it used to be 70+ cm/s two years ago). Caverject didn’t even give me an erection.

I also had angiography and an MRI, which didn’t show any damage to other veins or arteries - just the ones in my penis.

At this point, I feel like a penile implant might be my only solution. Has anyone else had a similar experience? Is it possible drugs damaged just the arteries in my penis? A year ago, I was still somewhat sexually active, but now even huge doses of PDE5 meds do nothing. I feel an empty, cold sensation in my groin, and my flaccid penis seems smaller than it was when I was a kid.

Any advice or shared experiences would mean a lot.

FrankTalk is a wealth of information. Hopefully you’ll find what you are looking for here.

Regardless of the reason for ED: Diabetis? HTN? Peripheral vascular disease? Venous leak? Arterial insufficiency? (seems to be your cause) the treatment is the same. Pills, VED+constriction ring, injections (monomix, bimix, trimix, QuadMix, and penile implant.

Some men try all of those. Some men rule some of them out from the start.

It seems like you have already explored pills and monomix. If you are willing to explore injections further, there are additional options.

Otherwise VED+constriction and penile implant are your options.

So explore the options and let us know how your progress goes.

On a side note: sounds like you are beating yourself up about all the partying you did when you are younger. Let go of that. None of us can go back. There are no do-overs. Besides, correlation does not equal causation. Just because you binged on drugs and got ED during the same time period doesn’t mean one caused the other. And there is no reason to think that stimulants attacked the vascular in your cock only.

But all of that is an academic exercise. The treatment is the same, so go for it and let us know what you do.

Hello mate. I'm sorry to hear about your situation. Let me start by saying that I had a similar experience. Albeit not as gradual as you. Mine was very sudden ED caused by mixing cocaine and viagra which caused priapism.

Cocaine and stimulants can absolutely damage Erectile function. They vasoconstrict the blood vessels involved with erections. Over time this can cause them to deteriorate if exposed to continous stimulant use. 'Coke dick'.

I unfortunately have no answers. I too have seen my penis atrophy because of damaged bloodflow to the penis. If you can continue to take tadalafil/cialis I would encourage it to maintain bloodflow and tissue. Discuss with your doctor first. Then start on the escalation of shots etc.

It's a real mental struggle. If you need to message me please reach out. I struggled with depression massively after my 'accident' as I now call it.
